This is to certify that the amount spent by ELIZABETH ANN

PALACIO who was assigned as medic who accompanied the
Enchanting Palo Brass Band during the International Woodwind and
Jazz Festival at the Cultural Center of the Philippines last July 26-31,
2016 was not included and paid from the fund advanced for the
aforementioned purpose per certified true copy of the approved
Project Proposal herein attached.
This certification is made to attest to the truth of the facts
herein set forth and for any legal purpose this may serve.
