I'm writing in relation to a letter I received on xx/xx/xxxx account number
xxxxxxx for a housing benefit overpayment/council tax benefit overpayment for the
sum of xxxxxx I'm writing to appeal your decision to reclaim the
overpayment as the overpayment has accured through no fault of my own
[Please state your reasons and include any evidence that will help with your case]

The stress it has put on my family life not just financially but emotionally too (make
it personal the stress they have caused)
I would also like you to place the account on hold pending my appeal outcome as I
don't think I should have to make any payments to this matter while
it's in dispute

I would also like to request a hard copy of all personal information you hold
details of the specific information you require and any relevant dates, for example:
- your personnel file;
- emails between A and B (between 1/6/11 and 1/9/11);
All call records between 'A' and 'B' (between 1/6/11 and
1/9/11
- your medical records (between 2006 & 2009) held by Dr C at D hospital;
- copies of statements (between 2006 & 2015) held in account number xxxxx .
