MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is made and entered into by and
between:
PALAWAN STATE UNIVERSITY TAYTAY CAMPUS,a public university with
principal address at Sorillo St., Poblacion, Taytay, Palawan,represented by its
Campus Administrator Mr. Severo E. Arzaga Jr. herein referred to as the
FIRST PARTY
-AND
NORTHERN PALAWAN PROVINCIAL HOSPITAL., a public hospital address
at Sitio Nagtacayan, Poblacion, Taytay, Palawan, represented by its Chief of
Hospital Dr. Ryan Clifford Estrada, herein referred to as the SECOND PARTY:
WITHNESSETH THATWHEREAS, the FIRST PARTY by its mandate, is empowered to establish a
partnership with other institution/ agencies as part of the medical
requirements/program of the University, to ensure the health of its students
and staff and to uplift the life of the people in the community.
WHEREAS, the SECOND PARTY promotes public health awareness upon the
initiative of Dr. Ryan Clifford Estrada with the support of all the NPPHs
doctors and medical practitioners and members of the staff by providing
basic medical services such as but not limited to consultation, laboratories
and other necessary procedures.

NOW THEREFORE, for and in consideration of the foregoing premises, the

parties hereby agree on the following:
1 That upon execution of this Agreement, the FIRST PARTY shall pay for
the amount of any medical procedure to be done depending on the
requirement of the SECOND PARTY in form of cash in Philippine
Currency or any other mode of payment as deemed necessary and
desirable as implemented by the SECOND PARTY;
2 That FIRST PARTY recognizes the availability of primary health services
within its domain and conducts an advance communication to the
SECOND PARTY on the above-mentioned services which are beyond the
means of the FIRST PARTY;
3 That FIRST PARTY and SECOND PARTY shall work hand in hand in
uplifting the health status of the universitys students and staff.
4 That this Memorandum of Agreement shall take effect immediately
upon execution and shall be binding as long as the party intends to

keep its mandate and may be terminated at the instance by any party
by serving a thirty-day (30) written notice for its termination.
IN WITNESS WHEREOF, the parties have hereunto set their hands this
30 day of June 2016 in Poblacion, Taytay, Palawan.
