AFFIDAVIT
I, GLORIA PASUSTENTO, of legal age, single, Filipino citizen and resident of
______________________, Bacolod City, Philippines after having been sworn in accordance
with law, hereby depose and say that:
1.

I was hired as Human Resources Manager (HR Manager) of San Carlos

BioPower Inc. (the Company) from July 1, 2013 to the present. As HR Manager,
I am mainly in-charge with the general supervision and administration of the
Companys human capital;

2.

It was a company policy that a performance appraisal is a condition to change the

status of an employee from probationary to regular employee. Based on the
report submitted to us, Mr. Larry Celestial has been apprised of the result of his
appraisal, and that the discussion was made on May 25, 201 as indicated in the
said report.

3.

On May 27, 2014, Mr. Celestial informed the office that he was terminated from
work effective May 26, 2014, but due to a series of meetings in the afternoon,
was told to come back the next day;

4.

On May 28, 2014, Mr. Larry Celestial came to my office informing me that his
probationary employment has been ended and I called Mr. Ronnel Bragais,
Biomass Manager, for the clarification. On that same day, I ordered the End of
Probation Period notice to employee and that on May 30, 2014, Mr. Celestial
came and signed the notice in the HR Office and was subsequently given a
Clearance Form to be accomplished.

On June 4 2014, I ordered the BPO to furnish Final Pay Computation of Mr.
Larry Celestial.

6.

Mr. Celestial submitted to us the Clearance Form on June 7, 2014. Upon

checking, admin clerk indicated in the remarks that a charger for an issued phone
had not been returned. On June 9, 2014, the issue has been settled clearing Mr.
Celestial of the charger, and that a mistake was committed in Clearance because
it was not updated for the return of the charger. With all things clarified, the
cleared was signed by me and later forwarded to the concerned department
heads.

On June 14, Mr. Celestial asks a copy of his clearance and check on his final pay.
HR informed him that his Clearance lacks signature from his immediate
supervisor and that HR will facilitate the completion and will release the
clearance after. Furthermore, we informed him that final pay computation has
been forwarded to the BPO and checks will be prepared for release once all
documentation has been completed.

In truth and witness whereof, I have hereunto set my hand this 6 th day of August 2014, in
San Carlos City, Philippines.
