I, MARIETA TAFALLA-CORNEL, of legal age, married and presently residing at
M. Belostrino Street,Brgy. Poblacion IV, Gen. E. Aguinaldo, Cavite, having been
sworn according to law, depose and say:
That I am born on September 08, 1955 at Gen. E. Aguinaldo, Cavite to
Dominga Dias and Natalio Tapalla, a copy of my birth certificate is hereby
attached;
That I am using Marieta Tafalla in all my documents, school records and
correspondences since my childhood;
That upon verification with the National Statistics Office, I found out clerical
errors in my name and the one registered is Maria Tapalla;
That both names pertain to one and the same person;
I am executing this Affidavit to attest the truth and for whatever legal purposes
it may serve and specifically for the purpose of processing and claiming as
beneficiary under insurance benefits of United Coconut Planters Life Assurance
Corporation.

TWO DISINTERESTED PERSONS
We, GERWIN BENCITO and VIRGILIO ERNI, both Filipinos, of
legal ages, married and residents of Real Street,Brgy. Castanos Cerca,
Gen. E. Aguinaldo, Cavite, Philippines, having been duly sworn in
accordance with law, hereby depose and state:
That we both personally know the person of JOWEL INCOGNITO
BELTRAN since his childhood because we lived in the same locality for a
long time, and his family has been a close family friend;
That we also know and we hereby declare that the said JOEL
INCOGNITO BELTRAN was born on March 9, 1961 at Gen. E. Aguinaldo,
Cavite, Philippines to PONCIANO IKAN BELTRAN and JOSEFINA
INCOGNITO;
That we are not related in any way, either by consanguinity or affinity, to
the said JOWEL INCOGNITO BELTRAN;
That JOWEL BELTRAN and JOEL BELTRAN is one and the same person:
As such, we execute this Joint Affidavit to attest to the truth of the
foregoing facts and for any other legal purpose that this Affidavit may
serve.
