B. Other Task/Functions
Performs other functions as may be assigned by their immediate superior
from time to time.
Attends in all official meetings.
Undertakes housekeeping and cleaning chores in work areas, observes,
and practices safety rules and regulations and cost reduction programs
(Performs 5S).
1. Medical Services
Administers treatment to employees who are not feeling well.
Administers first-aid treatment to injured employees.
Sends sick injured employees to hospital if the condition needs further
medical treatment immediately.
Advises sick employee to go home if the condition needs further rest at
home.
Assist the company physician during consultation time.
Issues medical and dental referral slips to employees.
Coordinates with the hospital and company dentist appointments.
Informs employees of health tips through reading materials made
available at the clinic, posting of information at the bulletin board.
2. Annual Medical Examination or Annual Dental Examination
Plans and implements the annual medical examinations of regular
employees.
Monitors employees who underwent annual medical and dental
examinations.
3. Pre- Employment Medical Examination
Processes the pre- employment medical examinations.
Furnishes the Human Resource Management Group copies of medical
examination results.
Process payment to Hospital.
4. Employee Compensation (EC) or Work- Related Accidents
Coordinates with the emergency hospital if cases need further treatment.
Prepares and monitors employee compensation applications
Prepares the accident report and issues reminder memoranda to
concerned employees and immediate superiors.
5. Social Security System (SSS) Related
Requires concerned employees to fill-up the Maternity Notification Form
and supporting document.
Issues Maternity Requirement Checklist.
Assist the employees infilling the Sickness Notification Form.
6. Sick Leave
Verifies reasons in sickness leave applications.
Issues return to work clearances for minor illness and maternity leaves.
Monitors sickness cases through patients consultation card, dispensed
medicine record and leave an availment report.
7. Clinic Records and Reports
Responsible for clinic equipment and supplies.
Maintains clinic records and reports and ensures that policy on retention
of said documents are implemented.
B. Periodic and Occasional Duties
1. Periodic
Prepares clinic reports such as:
Pregnant or maternity leave employees, Dispensed medicines, Illness
report, Department of Labor and Employment reports, Accident report.
2. Occasional
Replenish medical supplies.
Does clerical work in the Administrative Section.
Does other works as required by the company.
C. Authority
Decides when an employee needs to be sent home to rest or to be sent to
the hospital for further Treatment.
Authorized to use company vehicle for emergency medical purposes.
Authorized to accompany the employee to hospital or to residence if he or
she needs to be attended to during travel.
Issues Return to Work Clearance to employees for non- contagious illness
and maternity leaves.
