CBS Drama Diversity Casting Initiative

Participant Application

You may apply to participate in the CBS Drama Diversity Casting Initiative by completing and submitting this
Application.
Please note the following eligibility requirements must be met in order to proceed with the application process.
a) You must be 18 years old or age of majority, whichever is older in your State of residence as of the date
you submit your Application;
b) You must be a legal resident of the United States;
c) You must be a registered user of Vimeo prior to the submission of your Application; and
d) Neither you, your immediate family members (spouse, mother, father, sister, brother, daughter, son,
regardless of where they live) nor members of your same households (whether related or not) may be
employees, officers, directors and agents of CBS Broadcasting Inc., CBS Studios Inc., and/or CBS
Corporation and/or of any of their respective licensees, assigns, parents, affiliated and subsidiary
companies.
If you are invited to attend a Callback you must be willing to travel, at your own expense, to one of the
following locations on the dates specified:

ATLANTA, GA - November 7-9, 2016

AUSTIN, TX - November 3-4, 2016
CHICAGO, IL - November 2-4, 2016
MIAMI, FL - November 10-11, 2016
SAN FRANCISCO, CA - November 10-11, 2016

If selected for the Screen Test, you must be willing to travel to Los Angeles, California for up to five days and
four nights over the week of December 12, 2016. Travel for the Screen Test will be provided by CBS in
compliance with the Program Terms.
In addition to completing this Application, you must create a Video that records you performing one of the
monologues available here and then upload that Video and password protect it on the Vimeo website. The
Video should comprise two parts: (1) the slate; and (2) your performance of the monologue. At the beginning
of the Video, please slate your name carefully along with your height; the camera framing of the slate should be
a full body shot (i.e., a camera angle that records your entire body from head-to-toe). After the slate, you should
perform one monologue from the monologues available at the link above; the camera framing of the monologue
should be from your chest/shoulders up. Once you have created your Video, please go onto the Vimeo website
(www.vimeo.com) and follow the steps to upload and password protect your Video. Please see Program Terms
for additional details regarding your Video.
If anyone helps you film your Video, please have them sign the below Videographer Release and submit an
electronic copy of that along with your Application and link to your password protected Video.
Once you have completed this Application and you have uploaded and password protected your Video on the
Vimeo website, please email your completed and signed Application, the link to the Videos URL, the password
to access the Video, a headshot and resume, if available, and, if applicable, the below Videographer Release to
one of the below email addresses. Each email address corresponds to the Callback locations and dates listed
above. BY EMAILING YOUR APPLICATION AND VIDEO TO THE EMAIL ADDRESS LISTED BELOW

I am aware that
Participant is applying to be considered for participation in
the CBS Diversity Casting Initiative (Program). I am assisting Participant in the creation of an audition video
(Video) to be submitted in conjunction with Participants application materials to CBS Corporation (CBS).
I represent and warrant the following:
I agree that CBS shall exclusively own the Video and all elements thereof. Further, CBS shall have the sole and
exclusive right to exploit the Video in any and all media now known or hereafter devised worldwide, in
perpetuity, including not by means of limitation in and in connection with the Program and all promotion,
publicity, marketing or advertisement thereof. In the event that I appear in the Video, CBS shall have the
unlimited right to use my name, voice, and likeness (collectively Likeness) in connection with the Program
and any promotion, publicity, marketing or advertisement for the Program or any exploitation of the Program of
any kind. I grant the rights hereunder whether or not Participant is selected to participate in the Program.
I agree to forever release and hold harmless CBS, CBS Broadcasting Inc., CBS Studios Inc., and/or any of their
respective licensees, assignees, or parents, or affiliated or subsidiary companies, and the officers, employees and
directors thereof (collectively, Releasees) from any and all liability arising out of their use of the Video or my
Likeness. I agree not to make any claim against Releasees as a result of the use of the Video or my Likeness
(including, without limitation, any claim that such use invades any right of privacy and/or publicity and any
claims based on defamation, libel or false light or copyright infringement of any sort). I waive any moral
rights I may have in the Video. I agree not to post the Video or otherwise disseminate it in any medium
whatsoever, other than in connection with the Program.
Governing Law. This agreement shall be deemed to be entered into in Los Angeles County, California, and shall
be governed by and interpreted in accordance with the laws of the State of California applicable to agreements
executed and fully carried out within California.
Resolution of Disputes/Binding Arbitration The parties agree that any and all disputes or controversies arising
under or relating to this agreement or any of its terms, any effort by any party to enforce, interpret, construe,
rescind, terminate or annul this agreement, or any provision thereof, and any and all disputes or controversies
arising under or relating to the Video shall be resolved by binding arbitration under either the JAMS

streamlined or the JAMS comprehensive, as applicable, arbitration rules and procedures, except as
modified herein, in effect at the time the request for arbitration is made (the Arbitration Rules), at the
Los Angeles office or JAMS, or its successor (JAMS).
