GENERAL GUIDELINES

APPLICATION PROCEDURES

1. Only candidates for high school graduation of the current school year are qualified
to take the UST Entrance Test (USTET)

1. Applicants may apply on site or online (ofad.ust.edu.ph).

2. Submit the following together with the accomplished Application and
Grades Form:

Education Accreditation and Equivalency (NFEA&E)

GRADE DETAILS GUIDELINES

1. Only the School Registrar or Principal may certify the Grade Details portion of the form.
2. Compute for the average of all interrelated subjects taken up within the same school year.
e.g. [ Earth Science (85.00) + Science and Technology (87.00) ] / 2 = Science grade (86.00)
3. Record the final average grade for each year level in English, Mathematics and Science in the appropriate boxes.
4. State the grades in standard quantitative equivalent.

PEPT and NFEA&E passers, students who are graduates of previous school years, candidates for graduation from foreign schools
foreigners graduating from local schools must secure clearance from the UST Registrar's Office (Mondays to Fridays only).
5. Enrollment in any program or vocational course prior to the release of the USTET result shall lead to the nullification of the examination taken.
The fees paid for the USTET shall be forfeited in favor of the University.
6. Any misrepresentation of information written on this form, or any violation of the instructions given above shall be grounds for forfeiture of right
to enroll, or debarment in the succeeding semester if discovered in the current semester.
