that as you go along with your training you will know where to go.

Written Exam
This is your institutional assessment area which will measure your underpinning
knowledge and skills.
First I will administer to you the written exam. This measures you underpinning
knowledge.
It is a multiple choice type of exam. 15 items for 20 minutes please read and
follow the instruction carefully.

Demonstration
with oral
questioning
Now let us proceed on the demonstration of your skills. Given this materials and
equipment you are ask to perform _____________________ for 1 hour. And please be
reminded to observe safety precaution while performing the demonstration and
after the demonstration will proceed to the oral questioning and I will be rating

The primary purpose of this training evaluation is to identify areas for
improvement, for that reason please fill up what is outline in the training

Now trainees I would like to congratulate everybody for having been completed

and now you are qualified to undergo National assessment. We will be announcing
