PETITION
Petitioner, through the undersigned counsel and unto
this Honorable Court, most respectfully alleges that:
1. That petitioner is of legal age, Filipino, single, a resident
of 123 Teresa Street, Sta. Mesa Manila;
2. That respondent is the officer-in-charge, among others,
with the official records/ documents registered in the
City of Manila particularly certificates of live birth and
marriage contracts the office of which is located in the
Local Civil Registry, Manila City Hall, where it may be
served with summons and other legal processes;
3. That
petitioners
parents,
namely,
Miguel
R.
Ressureccion and Anita D. Ressureccion, were married
on January 15, 1978 in the City of Manila. A copy of the
marriage contract is hereto attached as Annex A;

4. That six months after their marriage, petitioner was

conceived by his mother. On April 15, 1979, herein
petitioner was born in Manila;
5. That in his Certificate of Live Birth under entry no. 4, his
name was erroneously entered as JUAN RESSURECTION
instead of JUAN RESSURECCION . Copy of the Certificate
of Live Birth of X is hereto attached as Annex B;
6. That petitioners surname is RESSURECCION and not
RESSURECTION. As reflected in his parents Marriage
Contract (Annex A), his fathers surname is
RESSURECCION and not RESSURECTION
PRAYER
WHEREFORE, premises considered, it is most
respectfully prayed to this Honorable Court that this instant
petition be GRANTED, that the erroneous name of the
petitioner JUAN RESSURECTION reflected under Entry No. 4
of his Certificate of live Birth be rectified and corrected to
JUAN
RESSURECCION
by
dropping
the
surname
RESSURECTION and changing it to RESSURECCION, his
actual and legal surname.
Other reliefs and remedies as may be available under
the premises are likewise prayed for.

Granting petitioner such other remedy or relief as this
Honorable Court may deem just and equitable in the
premises.

action or proceeding is pending in the Supreme
Court, the Court of Appeals, or any other tribunal
or agency;
6. If I should thereafter learn that a similar action or
proceeding has been filed or is pending before the
Supreme Court, the Court of Appeals, or any other
tribunal or agency, I undertake to report that fact
within five (5) days therefrom to this Honorable
