On or about the above stated date and time, the Defendant did

drive a motor vehicle while his (her) motor vehicle license was denied or revoked under Iowa Code Chapter 321J

I, the undersigned, being duly sworn, state that all facts contained in this Complaint and Affidavit, known by me or told to me by other reliable persons form the basis for my
belief that the defendant committed this crime
State all facts and persons relied upon supporting elements of alleged crime

On 12-12-2016 I was East bound on Main in Sac City when I noticed a white Ford Explorer West bound on Main. The driver Tammy Smith who I know
to be revoked. I made a traffic stop on Main and North 17th. Tammy was arrested and transported to the Sac County Jail for driving while revoked.
