PATHOLOGY OF THE CENTRAL NERVOUS SYSTEM


Microspecimens
68. Ischemic infarction in the brain (gray softening) (H.E. stain).
The earliest definitive microscopic signs of irreversible ischemic injury in the infract are
eosinophilic (red) neurons. It shows the loss basophilic staining (nucleic acids) and the
indistinctive cellular features. This process continues with increasing time so that all
distinct intracellular features are lost. One of the first changes after neuronal death (pink
neruons), is the pathy infiltration of inflammatory cells (green arrows), neurtrophils and
macrophages into the infarcted tissue.
45. Glioblastoma. (H.E. stain).
Histologically, it is formed by pleomorphic fusiform, polygonal or round cells, of
variable sizes, bizarre eosinophilic cells. The tumor is intensely cellularized. Giant cells
can be sporadically found. In some neoplasms, numerous mitoses have been detected, as
well as a certain tendency of perivascular palisade arrangement. Necrosis, hemorrhage,
cysts and edema are frequently found. At the periphery of necrosis, cells tend to arrange
in a palisade pattern. The tumor is well vascularised

Microscopically, the tumor is characterized by an uneven structure, having a complex
appearance of endotheliomatous and fibromatous areas that can predominate
alternatively, from one case to another. The tumor can be dominated by polygonal
meningothelial cells, fusiform fibroblasts or a mixture of the two. These structures can be
whorled or fasciculate, with the presence of small aggregates and calcified cellular
remnants, psammoma bodies. Reticulin and collagen are in variable amounts, and
cartilaginous and/or bony metaplasias are not excluded

Microscopically, metastatic neoplastic nodule consists of large polygonal cells (or spindle
cells) that have very pleomorphic nuclei, irregularly shaped and has abundant
cytopolasmic melanin pigment. Adjacent surrounding tissue is softened because of
ischemia.
