INSTRUCTIONS

Use this form to submit color matches by fabric (both labdyeings and First of Production). Use one Cover Sheet to identify all of the
information for several colors on a single fabric. Separate forms must be used for different fabrics. Only one fabric can be identified
for each form, unless the different fabrics are treated as GROUP SUBMITTALS (see the "Group vs Individual Approval" tab and
explanations on the color tabs). Only labdyeings can be submitted for GROUP approval. Production fabric must be approved
individually. Note that the term "Sample" refers to both labdyeings and First of Production submits.
1) Fill out the information in the sections called 1. JCPenney, 2. Fabric, 3. Finish, 4. Product, 5. JCPenney Instructions, 6.
Supplier, 7. Supplier Provided, 8. Colors from JCPenney, 9. Sample ID's, 10. For Resubmits, 11. Supplemental.
2) Then, print each individual request form (tabs labeled "Color 1" - "Color 28"). Note that each individual request form ("Color tab")
contains the information from this cover worksheet, so no additional input is required. Attach the color samples and send to the Color
Office.

3) Re-submits - Using the same form, update the SUBMIT #, the SAMPLE ID'S, and add the COLORCOMMUNICATOR RECORD #
from the rejected submit. Print the tabs for the colors being resubmitted, attach the new samples, and send.

THIS FORM MUST BE COMPLETED IN EXCEL. ANY HANDWRITTEN SUBMIT RECEIVED USING THIS
FORM WILL BE AUTOMATICALLY REJECTED.

What Brand is this for? Select one.
This is the official JCP ID for every fabric specified. Check with Sourcing for the correct number to enter.
Type in a general description of the fabric.
Select one of the constructions in the list.
Type in a fabric weight.
If this is a sweater yarn, type in yarn count details.
Select the fiber name. If a single fiber, enter 100 as the % in the next box and move on to describe the finish. If a blend, start with the fiber with highest %, move to
fiber #2, and enter the fiber with the next highest %, and so on until done.
Select the the second fiber name, indicate its percentage in the blend, then move to fiber #3 (if necessary).
Select the the third fiber name then indicate its percentage in the blend.
Select the the fourth fiber name then indicate its percentage in the blend. If there are more than 4 fibers, enter the rest in the comments.
If the mill will apply a finish, describe it here. If none, type NONE.
Is this finish supposed to change the color of the fabric?
Indicate whether this fabric will be used in a Discharge print. Note that the discharge printing process requires different dye selection criteria. This might effect the
accuracy of the color match.
Select one from the list so we know what it's for.
Enter the PPK#(s) when known.
If for trim, briefly describe where the trim goes.
Indicate whether or not this request is for a Test Program. This will impact timing and the number of submits allowable.
Type in any information received from the JCP Designer that will have a bearing on color approval.

Type in the Supplier Name. Supplier - make sure this is correct.


Type in the Supplier Number. Supplier - make sure this is correct.
Type in the Mill name.
Identify the company that is actually doing this color match.
Enter the name of the person identified as the second email contact, then enter the email address of the second person to notify with results.
Type in information about the submit, such as comments about the color match or any other relevant information that the Color Analysts should know.
Type in the color name or paste a list of color names (not to exceed 28) in this section. These must be the official JCP names.
Type in the color number or paste a list of color numbers (not to exceed 28) in this section.
Select which submit round this is.
Enter the ID name or number that will identify this color sample at the place where it was created. This ID must be mill-, factory-, or company-specific. Do not just
label it as A, B, C, or D.
If this is a resubmit, enter the ColorCommunicator record number from the .pdf rejection form received from JCPenney. SEND A COPY OF THE REJECTION FORM
along with the resubmit.
For the Supplier, enter the date that you send this submit to the JCPenney Color Office.
Select the type of sample being submitted.
Select NORMAL unless this is an FSRR (Final Submit Review Request) and you are requesting a BCD (Best Can Do - Override). Remember, FSRR's are not a good
thing.
Indicate whether this submit is for a specific fabric or a group of fabrics.
If submitting for Group approval, list all of the additional mill fabric ID's covered.
If submitting for Group approval, list all of the additional JCP fabric ID's covered.
