__________________________ hereafter referred to as TEACHER.

WHEREFORE, TEACHER represents that she is qualified able and willing

to render services to the SCHOOL and the latter offers employment in
accordance with the terms and conditions set forth in this Contract:
SECTION 1 - OFFER AND ACCEPTANCE
SCHOOL confirms TEACHERs engagement subject to the terms and
conditions of this entire Contract.
TEACHER accepts this engagement as stated in this contract and all
the terms, conditions and considerations that it contains.
SECTION 2 TERM OF CONTRACT
TEACHER is hereby appointed as PART-TIME TEACHER, for the SY 20152016, renewable subject to mutual agreement of the parties.
SECTION 3 DUTIES
TEACHER will perform the duties as may be ascribed in the duties,
responsibilities and expectations of SCHOOL and such other duties and
responsibilities as may from time to time be given by superiors
representing the SCHOOL.
In carrying duties and responsibilities, TEACHER shall comply with all
policies and regulations under the School Handbook/Manual and all
other rules as may hereafter be approved by the Board of Trustees.
SECTION 4 HOURS OF WORK
The TEACHER shall be required to render an _____-hour workday on a
__-day workweek from Monday to ______. Work done officially outside of
the school whenever necessary shall be considered as part of total
work hours.

As compensation for the services as TEACHER will receive a gross

WARRANTIES

CONDITIONS,

REPRESENTATIONS

AND

Adherence to Board Policy TEACHER represents and warrants that

he/she shall at all times conduct himself/herself in a manner consistent
with the policies of the SCHOOL as may be modified from time to time.
TEACHER acknowledges and agrees that she is responsible for reading
and being knowledgeable of the policies established by the Board of
Trustees of the SCHOOL.
Validity of Credentials TEACHER represents and warrants that all
evidence of experience and educational credentials he/she has
provided or hereinafter provides SCHOOL are accurate and valid.
TEACHER acknowledges and agrees that the falsification or alteration
of any documents evidencing such experience or credential shall be
grounds for immediate termination.
Mental and Physical Fitness TEACHER represents and warrants
that at the time of signing of this Contract, she is mentally and
physically fit and capable of performing normal professions duties.
Further, TEACHER warrants to maintain such good mental and physical
health and warrants to disclose immediately any communicable
disease, serious or life-threatening ailment.
Any breach of these representations and warranties shall be a ground
for termination of the Contract.
SECTION 7 - CONFIDENTIALITY
All records and documents of the TEACHER and all information
pertaining to its affairs are confidential and propriety in nature. Since
the positions of TEACHER at SCHOOL allow familiarity with such
confidential and propriety information, the unauthorized disclosure of
which to any and all third parties would materially damage the
SCHOOL, TEACHER agrees to hold such information other than for the
performance of services released by SCHOOL. TEACHER also agrees to
abide by all appropriate procedures to ensure the protection of the
SCHOOLs confidential, propriety and trade secret information.
SECTION 8 PERSONAL DATA
TEACHER hereby acknowledges, consents and agrees that SCHOOL
may keep records, personal information and data in its files or
computers and hereby further consents to the use and/or release of
such information by SCHOOL in accordance with the schools policy
regarding the issue and/or release of said information including release
of such information for accreditation, marketing and advertising
purposes.

SECTION 9 INTELLECTUAL PROPERTY

Any intellectual property prepared and produced at school direction or
resulting or arising from any partial or complete pursuit of the
objectives of this Contract, or part of the tasks performed for SCHOOL
under this Contract, whether actually used or not, shall become the
property of SCHOOL, unless a written agreement specifically stipulating
otherwise is executed.
SECTION 10 TERMINATION
This Contract may be terminated in any of the following ways:
a. Mutual agreement of both parties
b. Discharge for Cause Subject to substantial and procedural
due process, SCHOOL may terminate TEACHER services prior to
the expiration of her appointment for cause or as allowed by law
or labor regulations. Cause shall include but shall not be limited
to conduct which is prejudicial to SCHOOL or the morals of the
students, neglect of duty, breach of Contract, unsatisfactory
performance, insubordination, failure to follow a legitimate
directive of the President, gross misconduct, immorality, gross
inefficiency, incompetence, criminal acts, moral turpitude,
causing public scandal, any violation of the rules and regulations
of the school, and any violation of the terms of this Contract.
c. Resignation - hereby agrees that she/he shall not resign at any
time during the school year; or resign at any time without
observing the 30-day prior notice to the SCHOOL. For failure to
observe this requirement, the TEACHER hereby agrees to pay the
SCHOOL the amount equivalent to her/his one (1) month salary,
compensation and benefits.

SECTION 11 COMPLETENES OF THIS AGREEMENT

With the execution of this contract, the TEACHER hereby waives and
makes a quitclaim of whatever prior or existing rights, benefits,
interests or claims arising out of the previous or existing contract that
she/he may have or hold against the SCHOOL, its officials, officers,
employees or agents and for this purpose, the TEACHER hereby
represents and declares that she/he has received wholly and
completely by way of full and faithful satisfaction thereof such claim/s,
benefit/s, interest/s and /or other monies of whatever nature the
she/he may hold the SCHOOL responsible for.
This Contract including the attachment, if any, incorporates all of the
understanding between the parties and supersedes and expressly
terminates any prior agreement concerning employment with SCHOOL.
IN WITNESS WHREOF, the parties have hereunto set their
