The cause of the accident


Trying to vulcanize a tire of a crane used for lifting bridge beams, posted
between this and the mobile vulcanization station, he acted from on error,
the lever which triggers the fixing pillars system (in the ground). One of
the pillars grabbed his head and smashed to the ground causing death.
Worker fell from a height of 7 m and suffered a fracture of the spine,
which caused his death.
The used crane has maximum capacity to lift a load of 254 kilograms,
while precast concrete element weighed 310 kilograms, which led to its
overthrow on the parapet of the viaduct. Viaduct edges were not provided
with protective railing enough solid and high, so that the existing railing
gave way in the impact with the crane arm, causing imbalance and fall
down of the worker. He fell from a height of 22 meters while working to
mounting the viaduct curbs (with three other colleagues) and he died.
The worker suffered a pelvis fracture after he fell from a pillar of a viaduct
under construction, from about four meters high.

Machinery has performed a maneuver drive back and caught a worker.


He died instantly.
A concrete beam that was caught by a crane had to be fixed by the two
workers. Crane failed or crane driver gave a wrong command and
concrete beam broke away. This hit one of the workers, crushing him and
causing his death. The other, which was above the beam jumped to time,
for fear of being hit, from a height of about 12 m height, suffering multiple
injuries.
