Re: 2017 Lewis Elementary PTSA Scholarship

The J. Milton Lewis Elementary PTSA Scholarship Committee is pleased to announce


that we will be awarding three (3) $500.00 scholarships this year. We will be awarding
these scholarships to deserving seniors who attended Lewis Elementary School for at
least three (3) years.
Attached are copies of the Lewis 2017 scholarship package. Please distribute this
packet to any interested seniors.
Thank you in advance for your cooperation in providing a copy of the students
transcript, class rank, and SAT scores (or ACT score converted).
Three scholarships in the amount of $500.00 each will be awarded on March 2, 2017 at
the Lewis Elementary PTSA meeting at 6:45 PM. The winners attendance is
requested at this event. All entries must be postmarked no later than February 10,
2017.

Dear Applicant:
Attached is the application form for the J. Milton Lewis Elementary PTSA
Scholarship. This year, three (3) scholarships in the amount of $500.00
each will be awarded to graduating seniors who attended Lewis Elementary
School for at least three (3) years.
To be eligible to receive this scholarship, you must attend an accredited
college, university or trade school in the 2017 Fall Semester. You must also
have attended Lewis Elementary school for at least three (3) years of your
elementary education; please make sure to include your dates of
attendance in your application.
Due to the nature of the scholarship and the excellence that it represents,
we ask that you fill out the application accurately and thoroughly. Please
include at least one letter of recommendation from a significant adult in
your life (please make sure that this person is not a relative.) This letter will
attest to your character and will be used to support your application. Also,
please submit a typed 500-word essay (double spaced, size 12 font);
handwritten essays will not be accepted. Your essay should address the
following: your personal goals, career plans, and any other information that
may persuade the committee that you should be the applicant chosen.
Entries must be received in the Lewis Elementary office or be postmarked
no later than February 10, 2017.
Representatives of the Lewis Elementary PTSA Scholarship Committee will
review your application. All applications received will be assigned a number
and the individuals who review the applications will not know the students
identity while reviewing the applications. Applicants will be judged on
Academic Ability, Character, Community Service, and Extracurricular
Activities in both School and Community.

Rules and Requirements
The scholarship recipient must be a high school senior who attended
Lewis Elementary School for at least three (3) years of their elementary
education.
For the 2016-2017 school year, the Lewis Elementary PTSA will award
three (3) $500.00 scholarships.
A high school transcript must be submitted along with the original
application, one letter of recommendation and a 500-word or less
typed essay (double spaced, font size 12). Class rank and SAT scores
(or ACT converted scores) are also required.
The applicant must have at least a 2.5 grade point average.
The scholarship may be used for tuition/educational expenses in any
accredited college, university, or trade school in the fall of 2017. It
may not be used for living or personal expenses.
The scholarship recipient may accept other scholarships and financial
aid loans.
Payment to the scholarship recipient shall be in the form of a check
made payable to the approved school.
The final decision on awarding the scholarship shall be made by the
scholarship committee.
Additional letters of recommendation may be submitted up to a
maximum of three, one page each.
If a student fails to complete the semester or fails to enroll in school,
any refunds will revert to the Lewis Elementary PTSA.
Proper completion of the application and accompanying documents is
required. Incomplete applications will not be considered.
Recipients shall be selected without regard to race, religion, sex,
national origin, or handicap.
The Scholarship Committee shall base their final selection on the following
factors:
1. Academic ability and college aptitude.
2. Individual student involvement in the total school program and the
community.

Award recipients shall indicate acceptance of the Scholarship no later than


Wednesday, March 1, 2017. The recipients will be acknowledged at the
Lewis Elementary PTSA meeting on March 2nd, 2017 at 6:45 PM. The
recipients attendance would be greatly appreciated.
