1. Overall, how do you feel you are progressing?


I feel I am progressing well. I think I have been able to use and implement information that I
have learned from our E&I courses as well as Level 1 FW experiences. I am making progress
towards becoming a competent entry level practitioner, but I still have much to learn.

2. How are you doing with assignments, to include documentation?


I think my documentation is good quality, and I am getting faster at writing notes. However, it
still takes me longer to write a note than it does other therapists. One thing I struggle with is
point of services documentation. I have difficulty finding time or the ability to focus on
documenting while working with a patient.

3. Describe your current caseload to include the number of clients and your responsibilities for
managing your own caseload.
I will typically see 6 to 7 patients per day, but that number goes down to 5 if one of them is an
evaluation. I am responsible for designing the treatment plan for the day, carrying out the
treatment, and documenting. I have not had the opportunity to complete progress notes or
discharges yet, but I know that is coming in the next few weeks. Many of the patients I see
have suffered a fall, and that is the reason they are admitted. Other diagnoses include: TKA,
THA, dementia, and failure to thrive.

4. How do you feel you are doing with planning and implementing appropriate and meaningful
treatment for your clients?
I think I am doing well, because the big focus in this setting is on ADLs. I mimic the patients
home environment as best I can, and focus on what is important to them by asking the patient
what is important to them.

5. What are the most typical interventions you are doing at FW?
Bathing, dressing, hygiene, toileting, functional mobility, dynamic standing and sitting balance

6. How are you including your clients psychosocial factors into the interventions you select?
Particularly with patients who have suffered falls, I address their fear of suffering another falls
and the anxiety that goes along with it. I have used progressive muscle relaxation techniques
and abdominal breathing.

7. What have you done to share research on particular topics with your facility?
Because of the emphasis on fall prevention at my site, and my VIA topic focusing on fall
prevention, I been able to share information that I have found through my research. I have not
had to do anything formal, such as present, so I have mostly talked to other therapists about
what I have found in my previous CAPs.

8. Describe the nature (or health) of your relationship with your FW Educator.
I think I have a good positive relationship with my FW educator. She allows me to function on
my own, but she is still there for guidance or support when I need it. She still challenges me
each day to be a better student and therapist, but also knows what my limits are and where I
may need a little help.

9. How has your level of supervision been changing since week 3?

My FW educator trusts me more; she has given me much more freedom to plan and implement
my own treatments. She also trusts me to lead around a level 1 student and to be able to
answer their questions along the way.

10. Describe any unexpected or surprising feedback you received during the midterm review with
you FW Educator.
Nothing surprised me too much because her and I had talked about much of my strengths and
weaknesses during our weekly meetings.

11. Describe at least 3 achievements as well as opportunities for growth based on your midterm
performance evaluation.
Achievements: demonstrates positive interpersonal skills, responds constructively to feedback,
and collaborates with supervisor
Areas of growth: demonstrates effective time management, demonstrates understanding of
the costs and funding, and selects relevant screening and assessment methods.

12. Describe your plan to overcome these challenges during the last half of this FW experience.
1. To improve my time management skills, I will start to plan my day better and coordinate
treat times that do not interfere with doctors appointments or other therapies.
2. I will ask more questions about costs and funding and review my notes and readings to try
to gain a better understanding.
3. I will review that assessments available at the facility and their purpose to try to use them
with the appropriate patients.

13. Describe any other topic you would like to share or discuss with your faculty mentor.
None at this time.

14. Based on your responses above, reflect on your assessment of this fieldwork experience and
compare it with your original Professional Development Plan (PDP).
a. How are you progressing toward achieving the goals on your PDP?
I am making solid progress towards achieving them, and I think I created appropriate
goals that apply to both of my FW settings.
b. How are you linking your OTKE results with this fieldwork experience?
One area of my OTKE results I wanted to focus on is assessment and evaluation items. I
am learning and using new assessments at my current FW site.
c. Based on your answers to the above questions, be sure to make note if any goals/action
items on your PDP need to be modified. Your PDP will be revisited after returning from
both Level 2 FW experiences.
