Date: Monday January 30, 2017
My site supervisor had asked me to write up notes
about the daycare visit and PT session I observed
today.
I am going to make note on updates on the child the parent has
said, any observations made on the child, things discussed
during home visit, and plan for the next visit. I will follow the
template of what is to be taken note on about a home visit. I
will ask questions when needing clarification. The notes will
then be put on the YODA (site database) so the parents and the
EI can have access to it for referral.
Objectives:
1. The notes will help the EI and other professionals keep
themselves updated on the needs of their little peoples
cases so that they can better help with the childs
development.
2. The notes will help the parents stay informed on the
choices made on their regards of their childs development
and stay in the loop on all things concerning their child.
3. The notes will provide myself and the EI the opportunity
to reflect on their practice.
