PROJECT PHASE II (Previously Spanish-Assisted)

Status as of January 2017

The project is on-going with current project accomplishment of 24.37%. There is a slippage of-74.59% due to
continuous rain in the locality caused by northeast moonsoon and difficulty in sourcing of manpower. A 120-calendar
days time extension is on-process. There is an on-going clearing of landslides along the road going to the site, brought
by typhoon and continuous rain in the area making the access to the project unpassable. The hauling of steel bridging
materials cannot commence due to impassable road network going to the project site.

The project is under suspension with 10.26% accomplishment due to non-conformity of soil condition on the site. A
proposed another soil exploration shall be undertaken for the basis of re-design of the bridge structural component. A
proposed 60 days time extension is being processed.

Actual accomplishment of 14.96%. Only abutment B has been completed. The Contractor has no equipment for the H-
pile footing of abutment A. Suspension no. 3 has been approved to ROW. Final alignment to resolve Right-of-Way of the
project has been approved.

On going. Actual accomplishment is 90.20% with positive slippage of 1.20%. Activities include Cutting of Concrete at
Approach B, Assembly of Reinforcement at Approach Slab and Concrete Pouring at Approach A.

On-going. Actual accomplishment is 61.41% with positive slippage of 1.41%. Activities include embankment and
hauling of bridge component from DPWH Compound to Project Site.

construction materials (aggregates) to be shipped from the mainland (Sariaya, Quezon), as there are no qualified
sources within the locality. Bad weather causes rough seas, prompting the Philippines Cost Guard (PCG) stationed at
Atimonan Port to prohibit sea vessels to transport aggregates and equipment during heavy rains ans stormy days for
the safety of passengers. Request 30 C.D. extension approved up to Undersecretary Operation for Regional Operation

CD extension for Kinahulasan and 30 CD extension is upon approval and catch-up plans and revised S-curve were
asked from the contractor.
Status as of January 2017

Completed.

On-going with actual accomplishment of 25% and positive slippage of 21.09%. Activities include steel H-Pile driving
and fabrication and installation of rebars.

Actual accomplishment is 35% Completed activities include mobilization of materials, construction of facilities for
engineers, clearing and grubing, driving of test piles. Driving of regular piles is on-going.
Slippage to date is -2.40% due to delays brough by shortage of aggregate materials for RC piles. This is caused by the
strict regulation of Provincial Mining Regulatory Board for hauling of aggregates.

Actual accomplishment is 3% which includes mobilzation of quipment. To date, on-going activities include laying out
detour road, construction of facilities for the engineers and mobilization of equipment and materials.
UPMO - BMC coordinated with the Contractor to submit a catch-up plan for the incurred slippage.

The project is on-going with actual accomplishment of 65.5% and incurred a slippage of (-) 34.5% due to delayed
delivery of H-Piles and delivery of steel bridge components. The project was also suspended for 60 c.d. due to the
same reasons.

On-going with actual accomplishment of 93% and a slippage of (-) 7% due to RROW Acquisition due to widening of
access road to residential houses. As per site visit last July 04 - 07, 2016, it was observed that embankment works
were already finished and that the construction of riprap works are suppposedly installed last January and February of
2016. However, it was not evident at the site due to scarcity of boulders. The Implementing Office was advised to
double the effort in catching up to the schedule.
Status as of January 2017

The project is currently suspended due to the request for realignment of the Bridge structure and its approaches. As a
result of the request for realignment, the DPWH-CO Bureau of Design has conducted a site visit to furnish their
recommendations on the said project. The approval of the said request was then forwarded to the Regional Office as
part of their jurisdiction as per Implementation Arrangement duly approved by Undersecretary Romeo S. Momo.

The project was suspended due to the request for realignment of the Bridge structure and its approaches. A
geotechnical study on the relocation of abutments will be conducted as per discussion with the Project Engineer In
order to expedite the construction work and eliminate danger and inconvenience to the travelling public since the
existing bridge will be used as detour.

On-going with actual accomplishment of 83.06% with a slippage of (-)9.76%. On-going activities for this month include
launching of girders, construction of stone masonry and grouted riprap.

On-going with actual accomplishment of 93.62% and incurred a slippage of (-) 6.98% due to inclement weather. On
going activity includes installation of Item 610: Sodding (Zoysia Grass) in the approaches.

Actual accomplishment is 31.19% with a slippage of (-)54.11% due to slow execution of project schedule by the
Contractor. The bridge will be subject for realignment due to RROW problem. As-Stake Plan will be prepared for review
and approval of the Regional Office.

Project is suspended for 120 c.d. December 10, 2015 due to Road-Right-of-Way and Tree Cutting Permit issues. The I.O.
coordinated with the BOHOL 3rd DEO to conduct Parcellary Survey to list the affected Land Owners and
establishments. A permit for cutting is also on the process with the DENR as per DPWH Project Engineer.

Completed. The project was funded by the Provincial Government in the amount of PhP5,622,366.00.

execution of the project schedule by the Contractor. The proposed bridge is set to be realigned due to the presumptive
Important Cultural Property (ICP) status of the existing Tibyawan Bridge. Hence, once the As-Stake Plan and Parcellary
Plan for the realignment were approved, the construction of the bridge will resume.

(-)48.429% due to slow progress of work. The project was suspended effective March 09, for 87 c.d. due to RROW
problems such as improvements were encountered at the site and affected utility lines and electric posts. The on-going
activities include removal of existing bridge and installation of stone masonry and rubble concrete in the abutments.

On-going with actual accomplishment of 40.895% with a slippage of


(-)59.105% due to slow progress of work. The project was suspended effective March 09, 2016 for 87 c.d. due to
RROW problems and change in the alignment of the bridge and approaches to suit actual field condition. The on-going
activities include bored piling at Abutment A, excavation below water level and Embankment (Borrow).

On-going with actual accomplishment of 24.60% with a slippage of (-)04.925 % due to encountered RROW problem,
obstruction of structure and improvements within the construction limit. The project was suspended effective March
11, 2016 for 189 c.d. due to pending relocation of electric post DASURECO, relocated Water Pipelines of Sta. Cruz
Water District and on-going negotiations with residential houses obstructing the detour road.

On-going with actual accomplishment of 40.92% and incurred a slippage of (-) 9.38% due to peace and order,
inclement weather and RROW problem, obstruction of structure and improvements in the project site. The project was
suspended effective April 01, 2016 for 168 c.d. due to peace and order situation.

conditions at the project site and breakdown of heavy equipment. The Contractor was advised to render overtime
works during favorable weather condition to eliminate the negative slippage. On-going activities include installation
and launching of girder for the superstructure and preparation of gabions for slope protection.

On-going with actual accomplishment of 73.50% and incurred a slippage of (-) 8.17%. The on-going activities include
installation/erection of Centunion Modular Steel Structure and Accessories.

The project is on-going with actual accomplishment of 98.60%. The slow progress of work is caused by peace and
order issueOn-going activities preparation of base pouring of concrete pave shoulder. Already implemented and funded
under the GAA of CY 2015 of Sultan Kudarat 2nd DEO.
