PREFACE
Lewis Smith Dam is the largest earthen dam in the eastern United States. The dam, dedicated in May
1961 and put into service on September 5, 1961, is 2,200 feet in length and reaches a maximum
height of 300 feet. Construction began on November 25, 1957 and continued for four years. The dam
is owned and operated by the Alabama Power Company.
Located on the Sipsey Fork, the uppermost lake on the Black Warrior River, the dam creates Lewis Smith
Lake. The lake covers approximately 22,000 acres in Cullman, Walker, and Winston counties. The three-
fingered lake has 642 miles of shoreline and at full summer pool has a level of 510 feet.
At the national level, the federal government developed and published the Federal Guidelines for Dam
Safety in 1979. In addition, a Presidential memorandum was issued directing certain Federal agencies to
implement FEMA 93, Federal Guidelines for Dam Safety. FEMA 93 encourages strict safety standards in
the practices and procedures of Federal agencies and dam owners regulated by Federal agencies. The
guidelines state that Those charged with administering the guidelines must recognize that the
achievement of dam safety is through a continuous, dynamic process in which guidelines, practices, and
procedures are examined periodically and updated.
In 1996, the Water Resources Development Act of 1996 (Public Law 104-303) directed FEMA to establish
a National Dam Safety Program. The Act also directed FEMA to establish a National Dam Safety Review
Board whose purpose would be to advise the Director of FEMA on setting national dam safety priorities
and to provide assistance in monitoring State dam safety programs. The State of Alabama does not have
a Dam Safety Program.
The Cullman County Emergency Management Agency, in conjunction with other county organizations
concerned, initiated a program in May of 2015 to develop evacuation plans for affected areas
downstream from the Lewis Smith Dam. Due to the development of residential areas and numerous
other changes in Cullman County over the past several years, this plan supersedes other previous
versions, which should be destroyed.
Nearby towns include Colony, Dodge City, Hanceville, and Good Hope. Unincorporated communities
include Cold Springs, Bremen, Arkadelphia, Black Bottom, Valley Grove, Cates Bottom, and other rural
areas downstream within Cullman County.
This plan should be used in conjunction with the Cullman County Emergency Operations Plan dated
March 2013 (updated 2015).

EFFECTIVE DATE
This plan is effective immediately and will be activated by the Cullman County Emergency Management Agency
upon the direction of the chief elected officials of the affected jurisdictions.

BASIC PLAN
EMERGENCY EVACUATION PLAN FOR DAM FAILURE AND FLOODING
LEWIS SMITH DAM/MULBERRY RIVER AND TRIBUTARIES
I. PURPOSE:
The purpose of this plan is to establish procedures for warning, evacuating, and sheltering
persons who would be endangered in the event of the individual failure of the Lewis Smith Dam.
It also addresses similar emergency response actions which might be required in the event of
flooding, caused by emergency releases from the Lewis Smith Dam spillway, turbines, and
sluices. The plan is designed to accomplish these functions with minimum confusion and
maximum speed consistent with the Cullman County Emergency Operations Plan to save lives
and protect property.
II. SCOPE:
A. This plan is regional in application and pertains to possible failure or high discharge from
Lewis Smith Dam located on the Cullman-Walker County border of southwest Cullman
County. It describes the actions necessary to accomplish the warning and evacuation of
areas that would be inundated in Cullman County, Alabama. The plan addresses emergency
actions for Cullman County, Alabama only.
The total population estimate for residents was calculated for approximately 310 addresses
(identified from tax data) multiplied by 2.61 persons per household. The property value is
based on tax records for each parcel within the affected area.
B. This plan has been developed within the authority and guidance contained in the Alabama
Emergency Operations Plan (EOP) which governs responsibility and actions of state agencies
in an emergency or disaster situation. At the local level, the Cullman County Emergency
Operations Plan (EOP) is the overall response plan for all hazards.

IV. SITUATION:
A. The Lewis Smith Dam is located at river mile 443.3 above Mobile on the Warrior River near
Jasper, Alabama and is located 77.5 river miles upstream from the Bankhead Dam. The dam
is listed as the largest earthen dam in the eastern United States. Constructed between 1957
and 1961, the rock-fill dam has a crest at elevation 550 feet. The dam is 2,200 feet long and
has a maximum height of 300 feet. The crest width is 50 feet and base width is 1,060 feet.
The spillway is a 350 ft wide uncontrolled weir at elevation 522 feet. The lake surface area is
21,200 acres with a shoreline of approximately 642 miles. Full summer pool elevation is 510
feet; winter pool elevation is 496 feet. The lake volume is 1,390,000 acre-feet (approx.
452,932,890,000 gallons).
B. The probability of the failure of this dam with or without warning is extremely remote.
Any unsafe condition or situation would normally be detected early and remedial action
initiated as appropriate. If a controlled release from a spillway should be required or if any

possibility of dam failure was indicated, state and local Emergency Management Agencies
would be immediately notified by the Alabama Control Center (ACC).
V. CONCEPT OF OPERATION:
To assure appropriate and prompt response to an emergency situation at the Lewis Smith Dam,
the Alabama Power Dam Safety Division has classified potential emergency situation or
conditions according to the relative urgency as follows:
The APC Team Leader or designated representative will, in the event of any emergency, take
immediate action necessary to prevent failure and to prevent or minimize loss of life and property.
He/She will initiate emergency notification procedures based on conditions as follows:

1. Alabama Law Enforcement Agency (ALEA) Assist in warning, traffic control, and search
and rescue operations in coordination with local law enforcement agencies.
2. American Red Cross/Cullman Provide disaster relief functions in accordance with local
and state plans.
5. Alabama Forestry Service Provide disaster relief functions in accordance with local and
state plans.
D. Corps of Engineers
A. General.
Local warning and evacuation operations will be conducted by Cullman County and will
be carried out in accordance with procedures established in APPENDIX I of this plan. The
following guidelines apply to federal, state, and local actions in an emergency situation.

Voluntary evacuation may be recommended for residents who require assistance due
to lack of transportation and/or medical conditions. Evacuation may be recommended
or ordered by local officials as a precautionary measure if the situation continues to
deteriorate or dam failure appears imminent.
2. In a Condition A Failure is Imminent or Has Occurred Time has run out A warning
will be passed to local emergency management offices immediately. The State and local
Emergency Operations Centers will be fully activated.
Due to the extremely limited time available for dissemination of warning under these
conditions, the first indication of an emergency for many residents in the danger area
may be rapidly rising water.
This is particularly true from 0-3 miles below the Lewis Smith Dam. Under these
circumstances, endangered persons should move to high ground immediately and
await rescue. (Inundation Maps Page 37).
A. Time required for flood waters to recede to normal levels after dam failure will vary
depending upon distance downstream and the extent of failure. This time can be estimated
for a give location (river miles) by adding 48 hours to the peak time shown in APPENDIX 2,
page 18 of this plan.

B. The situation will be monitored by state and local authorities in order to determine when
residents may safely re-enter areas which were flooded. Clearance for re-entry will be
issued by AEMA in coordination with local governments based on recommendations from
the Department of Human Resources (DHR), the Department of Natural Resources (DNR),
and from the Mobile District Corps of Engineers.
Instructions concerning re-entry will be broadcast via the Integrated Public Alert and
Warning System (IPAWS) which currently includes the Emergency Alert System, NOAA
Alert Radios, and Commercial Mobile Alert Systems. These advisories will include safety
and health information provided by state and local authorities.

The Alabama Power Company announced at (Insert Time) today it had begun a controlled
drawdown of Lewis Smith Reservoir. An Alabama Power spokesperson said the lake levels
were being lowered as a precautionary measure due to indications of (Insert general
information).
The controlled drawdown is an effort to relieve pressure on the dam and prevent the
sudden and uncontrolled release of waters should a break occur in the structure. A further
objective of the controlled drawdown is to evacuate as much storage as possible in the

Lewis Smith Reservoir to prevent its failure should a failure at the Lewis Smith Dam occur.
Water is being released through the spillway and turbines at a rate that may increase
downstream river stages above normal flood levels, the spokesperson said.
Residents of low-lying areas should take appropriate steps including moving livestock
to higher ground and evacuating boats moored on Lewis Smith Lake and downstream.

This plan will be reviewed annually and revised as necessary by the Cullman County Emergency
Management Agency in conjunction with the Cullman County Local Emergency Planning
Committee (LEPC). Local stakeholders will be provided updates as necessary.

APPENDIX 1
I. PURPOSE:
The purpose of this appendix is to provide for the performance of functions or actions
specified in the Basic Plan and apply only to Cullman County.
II. SCOPE:
While this plan is regional, initial reaction to an emergency situation is a Cullman County
responsibility. Specific responsibilities and actions delegated to Cullman County officials
and agencies are addressed in this appendix.

Cullman County EMA will coordinate all activities involving actual operations,
including warning and evacuations.
4. Protective Action Decisions (PADs) will be implemented by local government with the
exception of a mandatory evacuation. AEMA will serve as a liaison between all state
agencies and local government operations, and will establish liaison with the Federal
Emergency Management Agency (FEMA) and the Corps of Engineers.
7. Each agency shall retain its autonomy during the operations and responsibility for
Protective Action Decisions (PADs).
As outlined in the current Cullman County Emergency Operations Plan, dated August 25, 2015:
P=PRIMARY AGENCY:
Responsible for management of the Functional Annex: Devise, coordinate, and implement
disaster recovery plans for the designated Functional Annex of the Cullman County Emergency
Operations Plan (EOP).
S = SUPPORT AGENCY:
Responsible to provide expertise, experience, and assets to the appropriate Functional Annex of
the Cullman County Emergency Operations Plan (EOP) as needed or requested by the Primary
Agency.

ESTIMATED POPULATION TO BE RELOCATED OR EVACUATED
Persons to be evacuated are located in the areas of Cullman County in the confines of ZIP
Codes 35077, 35057, 35055, and 35033. The areas affected are primarily located along and
south of Alabama Highway 69S, north and south of Alabama Highway 91 and west of interstate
65, exit 291.
Most evacuation studies indicate that, on the average, only about 40% or less of the
population, which evacuates an area during a disaster, will utilize designated shelter facilities.
Most of those evacuated will opt to stay with a friend or relative or stay at a hotel.
Cullman Countys shelters will accommodate a total population of 1293 persons and can
accommodate the anticipated 40% of the affected population, or a total of approximately 324
persons.
