GORDONS FUNCTIONAL HEALTH PATTERNS

HEALTH BEFORE HOSPITALIZATION:

DURING HOSPITALIZATION:
Patient is conscious and coherent. Concerns for her surgical
operation. Patient is willing to accept and listen health
teachings. Follow all the nurse instructions during her
hospital stay. Shows interest to recover easily and fast.

NUTRITION & BEFORE HOSPITALIZATION:

DURING HOSPITALIZATION:
Patient is on (DAT) Diet as tolerated. Client weighs 51 kg
and stands 55 tall.

Before operation, client being infused with an IVF of D 5LR


bottle # 5 @ 30 gtts/min hooked at her cephalic vein.

After operation, being infused with an IVF of PLR 1L @


30gtts/min as main line hooked at her left cephalic vein and
an IVF of PNSS 1L with side drip of 1unit PRBC hooked at her
right cephalic vein.
On NPO.
ELIMINATION BEFORE HOSPITALIZATION:
PATTERN Patient stated the she frequently urinates 4-6 time/day.
Patient is constipated and had a hard time in defecating
everyday.
DURING HOSPITALIZATION:
Doesnt experience difficulty in urination. Can defecate daily
due to intake of laxative.

Before operation, client was able to move her bowel


throughout the shift.

After operation, a foley bag catheter was used which


attached to urobag draining well with dark yellow urine.
ACTIVITY- BEFORE HOSPITALIZATION:
EXERCISE Akong exercise ra kay ang akong trabaho sa balay ug
PATTERN pagbantay sa tindahan as verbalized by the patient. She
does simple exercises on the upper extremities by means of
shaking and stretching. Able to perform ADLs.

DURING HOSPITALIZATION:
Before operation, patient can turn to sides with discomfort
due to pelvic pain and can ambulate with assistance.

After operation, patient is on bed rest, flat on bed shows


evidence of weakness. Steady and in deep sleep.
SEXUALITY- BEFORE HOSPITALIZATION:
REPRODUCTION Patients had her menarche when she was 14 years. Patient got
PATTERN pregnant on her first baby when she was 20 years old, delivered
preterm twins. She usually had her menstruation 1 weeks or
more and consumed 6 soaked pads per day. Patient diagnosed
with Right Breast mass @ 2005.

DURING HOSPITALIZATION:
Patient complaint of pelvic pain and undergone TVS and was
diagnosed of Multiple Uterine Myoma.
SLEEP-REST BEFORE HOSPITALIZATION:
PATTERN Matog ko igka human nako og paninda as verbalized by the
patient.

DURING HOSPITALIZATION:
Patient cant sleep properly due to pelvic discomfort and noisy,
not well ventilated environment.
COGNITIVE- Speech clear without slur and stutter. Follows verbal cues.
PERCEPTUAL Express ideas and feelings clearly and concisely. Stated she can
PATTERN recall past events. Patient is able to see without any help of
visual aid. Patient has no problem with perception.

Before operation, patient verbalized Sakit kayo akong pus.on.


Magbalik balik ang sakit. She rated pain as 8 from a pain scale
of 1-10. She claimed that pain radiates at hypogastric pelvic
region.

After operation, Akong tahi nagsakit pa as verbalized. Felt


pain around the surgical site at hypogastric region. Slight facial
grimace noted. Able to respond to stimuli verbally and
physically with weakness noted. Believes that pain felt due to
post- operative experience.
ROLE AND Patient has a good relationship towards own family. Satisfied
RELATIONSHI with the role as mother. Patient adjust to the situation and is
P dependent towards the nurse. Still plays the role of a mother
despite of her condition.
SELF BEFORE HOSPITALIZATION:
PERCEPTION Describes self as a normal person. Loving herself as a mother to
SELF her children. She focuses on her children.
CONCEPT
DURING HOSPITALIZATION:
Patient still manages to appear calm and relaxed. Agreed to be
operated and undergone surgery and gives her trust to surgical
team.

Before operation, she prays all the time. She desires that no
complications will arise after surgery.

After operation, claimed that her major concern is her fast


recovery.
COPING- BEFORE HOSPITALIZATION:
STRESS Copes up with stress by doing household chores and taking a
TOLERANCE nap or sleep. Patient claimed that she copes up with problems
by talking about it with family and finds ways to resolve it
together.

DURING HOSPPITALIZATION:
Patient stated that her major stressor was the condition she is
facing now and copes up by talking to her family members
during visitation hours. Verbalizes desires to recover fast and
able to accept situation by cooperating with the advices and
procedures.
VALUES AND BEFORE HOSPITALIZATION:
BELIEF Patient is a Roman Catholic goes to church every Sunday
together with her family.

DURING HOSPITALIZATION:
Patient had no restrictions in the surgical procedure brought by
religion. The admission and surgery dont interfere with spiritual
practices.
