Subject- Show cause notice issued Under Bank Staff rule

Dear Sir/Mam,

During my posting at Jaipur- Main Branch As priority Relationship Manager Certain


irregularities has been observed pertaining to Misappropriation of fund. Below is the case
wise Explanations against the charges levied against me.

1. First instance was of one of my Priority customer Name Mr. Gopal krishna bhargava in
which there were 2 FD which has been liquidated and it has been alleged that 3
cheques (683692+683693+683694) aggregating 158000/- has been self withdrawn
by me without customer awareness, with this would like to add that agree that cash
withdrawn has been initiated but with the customer consent.
But since customer was to go through Knee transplant surgery the cash has been lied
up with me, as was not able to get in touch with customer, for which customer was
fully aware that the cash is lying with me.
Also yes customer approached the branch but for debit card issuance, which has
been issued.

2. Second instance reported was of Mr. Abu talib in which it has been stated that the
cash withdrawn of 200000/- along with FD breakage of 217000/- has been done
without customer awareness with the Deactivation of SMS alerts
Yes buy the point but customer daughter itself has handed over the FD receipts to be
liquidated and also been admitted by the customer that 200000/- cheque has been
issued to me for making the FD.
Also regarding the SMS Deactivation would like to add that customer daughter has
asked me to change the number for which request and requsition has been given to
her and has been handed over to me by her only.
So when the cheque of 200000/- has been issued by him itself only as also well
admitted by customer itself then Forged signature cannot be done. Also regarding the
FD liquidation as stated earlier the FD receipts has been handed over by customer
through her daughter.

3. Third instance was of one of our prime salary customer Mr. Ratan Singh solanki
Ratan singh solanki is father of my friend Rupashi solanki . Since long Mr.Solanki was
visiting the branch for issuance of debit card and loose cheque book as his home
construction was going on, and has applied for the loan in Indian railways for
construction. After visiting so many to time no one issued the instant debit
card+cheque book and has diverted to DBC channels, but since Mr.solanki lives in
outer area the cheque book applied through DBC didnt got delivered. For this Mr.
solanki has asked me to issue a new loose cheque book urgently for payments which
has been done on priority treating him as Friends father post the customer
instruction.
But since Mr. Solanki is TT in Indian railways and keeps on travelling issued 2 cheque
to me for the self withdrawn part. But somehow there were some confusion
happened between Him and her daughter which later got resolved.

4. Forth instance was of Mr. Sanjeev Gupta who is one of our priority customer in which
customer was having few mutual fund investments which was to be matured off.
For which being a RM approached and suggested the same and taken the investment
cheque as per his knowledge.
It has been reported that have tried to withdraw those cheques by self mode through
other branches wherein it was totally in knowledge of his PA since use to deal with
her for any kind of banking related work.

With all the above explanation buy the point the negligence has been happened thinking of
doing service to the customer and accept the fact have point out banks Code of conduct and
trust towards its employee.

But while going deeply through each case believe that would be able to understand the
point of just giving service to customer led to misappropriating of funds.

But since there was not Mollified intention in each case requesting you to please consider
my sincere apology and also assuring the same that will not happen further in career with
axis bank.
