ACCIDENT WAIVER AND RELEASE FORM

I hereby assume all of the risk participating in PPINTL Combine including by way of
example and not limitation , any risk that may arise from negligence or carelessness
on the part of persons or entities being released from dangerous, or defective
equipment, or property owned, maintained or controlled by them, or because of
their possible liability without fault.
I certify that I am physically fit and have not been advised to not participate by a
qualified medical professional. I certify that there are not any health related reasons
or problems that preclude my participation in this combine. I acknowledge that this
accident waiver and Release of Liability will be used by the organizers of this
combine in which I may participate and that it will govern my actions and
responsibilities at said Combine.
In consideration of my application and permitting me to participant in this combine,
I hereby take actions for myself, executors, administrators, heirs, next to kin,
successors, and assign as follows
(A) I WAIVE, RELEASE, DISCHARGE from any and all liability including but not limited
to, liability arising from the negligence or fault of the entities or persons
released, for my death, disabilities, personal injury, property damage, property
theft, or actions of any kind which may hereafter occur to me including my
travelling to and from this Combine. The following Entities or persons: Premier
Players International PPINTL, Brandon Collier, representatives or volunteers at
this combine.
(B) I Indemnify, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or
persons mentioned in the above paragraph from all liabilities and claims made
as a result of the participation in this combine, weather caused by negligence or
otherwise. I acknowledge that this combine may carry the potential of death,
serious injury, and personal lost. The risk my include but are not limited to, those
causes by terrain, facilities, temperature, weather, condition of participants,
equipment, traffic, actions of other people including, but are not limited to,
spectators, fans, participants, volunteers, coaches, lack of hydration. I consent
and agree that PPINTL and or their coaches, representatives, volunteers may
take photographs and digital recordings or me and participants during the event
and use them in any and all media for training or promotional purposes. I further
consent that my identity may be revealed therein or by description text or
commentary. I waive any rights, claims, or interest and I understand there will be
no financial or other remunerations. The accident waiver, release of liability and
image release shall be contrued broadly to provide a release and waiver to the
maximum extent permissible under application law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS
CONSTENT I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT
AND I SIGN ON MY FREEWILL.
