Please see reminders and guidelines below for preparing

your send-out specimen(s) to MakatiMed:

1. Only white and blue SHAP forms should be forwarded to MakatiMed; the green form serves
as partners copy.
2. In accomplishing the forms, please write legibly and in print.
3. Indicate the complete address of the patient on the Address space provided.
4. Write the full name of the patient including the middle name.
5. Kindly double check if the physicians request matches the patient identifiers on the SHAP form,
particularly the patients date of birth and age.
6. Do not forget to indicate the following details in the SHAP form:
a. Date and time of collection
b. Patients last meal (if test[s] requires fasting)
c. Last dose of medicine intake (Eg. Phenobarbital & Vancomycin)
d. Patients extraction position (for Nuclear Medicine tests)
e. Type of specimen and if from what tap (test tube)
f. Storage temperature (if specimen was not sent on the same day of collection)
g. Mode of transport (if specimen is on ice)
7. The specimen tube container must be labeled with the following:
a. Patients complete name including middle name
b. Patients date of birth
c. Type of specimen
8. Lastly, please observe proper packaging of specimen:
a. Use a screw cap receptacle or securely tape the cover or cap of the specimen tube to prevent
from spillage and leakage.
b. Place the specimen tube inside a resealable plastic bag or in clear plastic before placing
inside the primary packing material.
