Title of Community Health plan:

Problem Identified:
The problem identified in the municipality of Sta. Ana is the high
prevalence of upper respiratory tract infections (URTI) among the
residents. It affected 9,452 individuals in 2009, which comprise 15.52%
of the total population of Sta. Ana, and it also has the highest
morbidity rate.

Goal: Since the high prevalence of upper respiratory tract infections is

the identified problem by the group, the goal of the community health
plan is to decrease the prevalence of URTI among the residents of Sta.
Ana.

General Objective: The general objective of the community health

plan, {___________} is to raise awareness on the true nature of upper

respiratory tract and to promote simple lifestyle changes that may

reduce the cases of upper respiratory tract infection in the community

by the end of May 2017.

Specific Objectives
1. To broaden the knowledge of the residents about the true nature of
upper respiratory infections- its causes, symptoms, chronic effects and
preventive measures.
2. To maintain the information imparted and to promote long term effect
of a good hygiene through proper hand washing
3. To motivate the residents to have a more active lifestyle through
proper exercise so as to decrease the prevalence of URTI in their
community
