If you would like your child to take the free, optional bus to and from the 5k, please indicate
below. Additional riders that would like to ride the bus can do so at a cost of $8/seat.
Additional Riders include the following: parents/guardians, teachers, running buddies,
principals, family members, etc. *Please Note: small children in car seats are required to pay
the addition fee as well. If an additional rider will be accompanying your child, please specify
and submit payment with this form.

PLEASE NOTE:

Buses are scheduled to arrive to the 5k site 60-75 minutes before our teams start
group.
The pick-up/drop off location may NOT be our school/site but will be within a five (5)
mile radius.
