Critrios para retratamento quando em regime PRN(a

critrio medico)

Treat if there is persistent or recurrent subretinal or

intraretinal fluid on OCT, unless there has been no decrease in
fluid after 3 consecutive monthly injections. Treatment may be
reinstituted at a later visit if there is increased (relative to the
visit when treatment was stopped) fluid on OCT.

Traduzindo

Tratar se existir fluido intra ou subretiniano persistente ou

recorrente no OCT, com a exceo se no houver regresso do
fluido aps 3 injees mensais consecutivas. O tratamento
deve ser reinstituido em visita posterior se houver aumento de
lquido ao OCT.

A DUT segue este conceito e este no deve ser mal

interpretado. A DUT no diz em nenhum momento que se deve
tratar somente 3 injees consecutivas. E tambm no diz que
se houve melhora na 3 injees que no se deve tratar mais. A
DUT diz somente que se no houver nada a melhorar, no
PRECISA de nova aplicao(SEGUINDO O PROTOCOLO PRN
PRONTO que foi seguido no CATT tambm). A DUT diz tambm
que se no houve melhora alguma aps 3 aplicaes mensais
consecutivas o tratamento PODE ser paralisado, MAS como
vemos acima, no ter MELHORA no significa PIOR. E se houver
sinais de PIORA o tratamento deve ser prontamente
reinstituido.

(Juntando dados dos principais estudos IVAN e CATT alm do

PRONTO que teve seus dados contestados e ao final mostrou
que tratamento mensal fixo por 24 meses tinham melhor
resultado que PRN nos moldes especificados pelo protocol
PRONTO)

Giving injections continuously monthly for 2years on a PRN

regimen with strict monthly monitoring using SD-OCT and
retreatment, whenever any evidence of fluid is noted by retinal
imaging has been found to be the regimen that secures the
optimum results in vision outcome.

Mais uma vez as opes em TODO mundo so: dar injees

mensais por 2 anos ou PRN com monitoramento por OCT
mensal independente do quadro melhorar, e retratamento se
ocorrer a evidncia de QUALQUER fluido retiniano para
assegurar os melhores resultados visuais possveis.

Em nenhum lugar dito para tratar 3 injees mensais e parar

se ainda houver fluido subretiniano. Se existe melhora
progressive o tratamento deve continuar at que o lquido se
esgote. Se aps 3 injees consecutivas o lquido no involuir
mas ficar estvel, considera-se leso crnica e paralisa-se o
tratamento. At que ocorra sinal de mudana com piora do
quadro.

At mesmo o estudo PRONTO que foi o precursor do tratamento varivel, e que teve
diversas falhas apontadas ao longo dos ltimos 10 anos, e que assume critrios no
to rgidos para tratamento preconizam:

These criteria were a loss of five letters or more on a standardized visual acuity
chart with evidence of any subretinal fluid on OCT, an increase in central retinal
thickness of 100 m or more on OCT, any evidence of new hemorrhage; new classic
CNV, or detection of fluid on OCT persisting 1 month after an injection.

Ou seja, mesmo neste protocol a persistncia de fluido(sub ou

intraretiniano) no OCT aps 1 ms de injeo critrio para
reinjeo. E encaixa-se no que a DUT refere a melhora,
estabilidade ou piora do quadro.
