What is School Transition?
SchoolTransi onisaprocessthatpreparesallpartnersstudents,families,
schoolsandcommuni estodevelopknowledge,skills,andrela onships
thathelpstudentsmovefromoneeduca onalse ngtoanother.

Inthisinstance,studentsaretransi oningorchangingfromelementaryschooltomiddle
school.Thetransi onfromelementarytomiddleschoolisachangethatimpactsnotonlystu-
dents,butalsotheirfamilies,schoolsta,andthecommunity.
Schooltransi onisaprocessthatinvolvesfourfacets:
Readyschools;
Communitypar cipa onandsupport;
Familyknowledgeandinvolvement;and
Schools/educa onalse ngscommi edtopreparingchildren.
Schooltransi onisnotaonesizefitsall
approach,butaframeworkthatincludes
strategiesaswellaspoliciesandprocedures

incorporatedasaworkgroupwithinexis ngcommi ees

coordinatormaybetheassistantprincipal,school

theac vi esasneededtomeetthelearningstylesofstudentsandthedesiresoftheiroverall

duringthe4thgradeyear,perhapsinthespring,andcon nuethroughthefirstsemesterof
the6thgrade.Thissugges ona emptstoavoidthecurrentprac ceinwhichelementary
schoolsu lizethela erpartofthe5thgradetoaddressmiddleschooltransi onconcernsor
issues.Studentsandtheirfamilieswillreactbe ertosmallerdosesofinforma on,andthe
variousongoingac vi eswillclarifyandsolidifyinforma onforall.
Asyouprogressthroughthetoolkityouwillno cethatitisdividedintothreegradelevels:
4th,5th,and6thgrade.Foreachgrade,therearenumerousac vi esthatcanbecompleted
bystudents,schools,families,orcommuni es.Pleasenotethatthistoolkitdoesnotaddress
thevaryingacademicabilitylevelsof4th,5th,and6thgradestudents.Therefore,teachersor
schoolstashouldadjustinstruc onasitisrelatedtotheseac vi estomeettheneedsand
abilitylevelsoftheirstudentsandfamilies.Toincreaserigor,itisalsorecommendedto
includehigherorderthinkingskillsorques onsappropriateforeachac vity.Theuseof
