AFFIDAVIT OF LOSS
I, ILDEFONSO C. OCHEA SR., Filipino, of legal age, married and a resident
of Fatima, Payao, Zamboanga Sibugay Province, Philippines, after having duly sworn to
in accordance with law, do hereby depose and say, that;

1. I am the same ILDEFONSO C. OCHEA SR who purchased a parcel of land from

ARTEMIA GALLEROS-LOW, covered by Transfer Certificate of Title (TCT) No. T-
26,501, and situated in the City of Pagadian, Province of Zamboanga del Sur;

2. While processing the transfer of the title under my name, the Owners Duplicate
Copy of TCT No. T-26, 501 was lost and nowhere to be found;

3. Despite diligent effort exerted to locate the owners duplicate copy of the said
title, the same was proven futile and the said title remained lost and nowhere to be
found;

4. I am executing this affidavit to attest to the truth of the foregoing statements to

support for the replacement of the said owners duplicate copy and for any legal purpose
this may serve;

JOEDEL UY, of legal age, married, Filipino and a resident of Pagadian City,
Zamboanga del Sur, to be my true and lawful Attorney-in-Fact, for me and in my name,
place and stead, to do and perform the following acts to wit:

1. To cause the annotation and/or recording of the Affidavit of

Loss, involving TCT No. T-26, 501, before the Office of the
Registry of Deeds, Pagadian City;

2. To follow-up, get and/ or receive the certified true copy of TCT

No. T-26, 501, where the above-mentioned Affidavit of Loss is
already recorded or annotated at the back of the said title;

3. To perform all other acts that relate to said representation;

4. To sign any document relating to the powers herein granted.

HEREBY GIVING AND GRANTING, unto my said Attorney-in-Fact full

power and authority whatsoever requisite, necessary or proper to be done in and about
the premises as fully to all intents and purposes as I might or could lawfully do if
personally present, and hereby ratifying and confirming all that my said Attorney-in-
Fact shall do or cause to be done by virtue of this present.
