Specific Goal: My audience will learn the importance of medication therapy management for patients and
how it can impact their health.

Application: My audience will be better educated of what medications are prescribed and how they affect
them and their loved ones.

Ethos: Primary Ethos: I am in contact with patients daily who I can save from potentially harmful medication
interactions and barriers for medication adherence.

Secondary Ethos: I will cite _4__ references out loud in my speech.

Pathos: Sharing examples of medication issues I have helped address could have saved their grandmother
or grandfather, sister or brother, mom or dad.

reduce unnecessary trips to the Emergency Room.

occurred had no one addressed the issue.

Hook: Did you know? The American Pharmacist Association MTM website states: Medication-related
problems and medication mismanagement are a massive public health problem in the United States.
Experts estimate that 1.5 million preventable adverse events occur each year that result in $177
billion in injury and death. (AphA MTM Central, 2017)

III. MTM is when a pharmacist speaks with a patient who qualifies based on CMS guidelines, to
identify drug related interactions and adherence barriers and my help determine appropriate
solutions.
A. Based on CMS 2017 guidelines, eligible patients are required to have at least 2-3
chronic diseases such as asthma, diabetes, hypertension and dyslipidemia, taking 2-8
chronic medications, and are likely to incur annual costs for covered Part D drugs
greater than the 2017 threshold of $3,919. (Jennifer R. Shapiro, 2016)
B. Work with patients healthcare providers to get better Outcomes.
a. Outcomes can involve cure of disease and or ease of symptoms, slow or stop
progression of health condition.

Transition: To get better Outcomes we as a pharmacy team need to identify those patients and refer them to
our pharmacist. According to Cardinal health company 2016 MTM Trends Report on outcomesmtm.com
Approximately 56% of MTM programs target beneficiaries who have filled at least eight covered part d drugs.
(Health, 2016).
Dr. Mercola stated in a 2011 article-
Just over 89 percent of Medicare patients take prescription medicine daily
46 percent take 5 or more prescriptions chronically
Nearly 54 percent take meds prescribed by more than one doctor (Mercola, 2011)

IV. My fellow technicians and I spend our work days detecting and following up on patients and
their providers that need or have had a MTM session with our pharmacist.
A. We locate patients within an adherence list and med reconciliation referrals under those
previous guidelines.
B. After the pharmacist has focused on the medication issue/barrier we follow up with the
physicians and patients for any other concerns.

Transition: Once we have targeted the patient we need to let them know they can utilize MTM
to their advantage.

V. Patients who choose to take advantage of our help and utilize an MTM session, will have a
better understanding of their health and what benefits can help.
A. It will help them remember why and best times to take medications for best Outcomes.

b. Can save time/money at ER and cost of drugs.
c. Patient providers can work collectively to ensure quality care.

Transition: Now that these patients have been assisted and educated, they can be more
adherent to their health and well-being.

professionals to give you the best care.
