Greetings!

We would like to ask your persmisson to allow us, students of Management

Research, under the supervision of Professor Mayeth Salandanan, to conduct a
survey in our school. This is in view of our thesis, entitled, Factors affecting the
academic performance of college working students of the University of Perpetual
Help System Molino. We are conducting the survey among students in the
University. Attatched herewith is the survey questionnaire to this study.

The survey would be conducted to students randomly selected inside the presmises
of University of Perpetual Help System Molino. Participation of respondents in the
survey is entirely voluntary. All information provided by the respondents will be kept
in utmost confidentiality and would be used only for academic purposes.
