UriageSPF50+cream.After4days,apositivereactionwasobservedtothecream(++).Themanufacturersuppliedthe
individualingredientsoftheirproduct.Outofthe24componentsprovided,onlytetrasodiumethylenediaminetetraaceticacid
(EDTA)0.2%aquawaspositiveanditwasnegativeinfivecontrolssubjects....Inourpatient,therewasacloserelation
betweentheapplicationofthesunscreenandtheappearanceofeczema.Positivepatchtestwasonlyobtainedwith0.2%aqua
tetrasodiumEDTAasacomponentofthesunscreen,whereasethylenediaminewasnegative.Hiseczemaresolvedoncethe
sunscreenceasedtobeused.HeisnowusingamineralbasedproductwithoutEDTA.

toxicity.

years),and1g/kgininfantslessthan1yearold.
E)GASTRICLAVAGE:Considerafteringestionofa
potentiallylifethreateningamountofpoisonifitcan
beperformedsoonafteringestion(generallywithin1
hour).Protectairwaybyplacementintheheaddownleft
lateraldecubituspositionorbyendotracheal
intubation.Controlanyseizuresfirst.
1)CONTRAINDICATIONS:Lossofairwayprotectivereflexes
ordecreasedlevelofconsciousnessinunintubated
patients;followingingestionofcorrosives;
hydrocarbons(highaspirationpotential);patientsat
riskofhemorrhageorgastrointestinalperforation;and
trivialornontoxicingestion.
F)WHOLEGUTLAVAGEHasbeenutilizedasalternative
methodforGIdecontamination.Theuseofwholegut
lavageinthemanagementofpoisonedpatientsis
currentlycontroversial.Potentialsolutionsforuse
includeColyte(R)andGolytely(R).
G)INTRAVENOUSENVENOMATIONPROCEDURES:Thereislittle
thatcanbedonetoeliminateexposuretotoxins
administeredintravenouslyorbyenvenomation.
Proceduressuchasfasciotomyandfasciotomyare
recommendedwhendocumentedpressuremyolysisis
functionthreatening.
H)GENERALTREATMENT
1)Establishrespirationandcreateanartificialairway,
ifneeded.
2)Treathypotensionwithfluidsavoidvasopressorsif
possible.
3)Pulmonaryarterycatheterizationshouldbeconsidered
inpatientswithpersistenthypotensiontooptimize
utilizationoffluidsandvasopressors.
4)HYPOTENSION:Infuse10to20mL/kgisotonicfluid.If
hypotensionpersists,administerdopamine(5to20
mcg/kg/min)ornorepinephrine(ADULT:begininfusionat
0.5to1mcg/min;CHILD:begininfusionat0.1
mcg/kg/min);titratetodesiredresponse.
5)Aggressivelytreatandevaluatecomaregardlessof
suspectedcause.Intubateandventilateasneeded.
Comatosepatientsshouldreceiveoxygen,naloxone
(Narcan(R)),thiamine(adults)andeitherD50orrapid
determinationofglucoselevel.Checkcoretemperature
toevaluateforhypoorhyperthermia.Consider
evaluationforCNSlesionorinfectionwithCTand
lumbarpuncture.
6)Performserialexaminationstodeterminewhetherthe
patientisimprovingordeteriorating.
