INTERN AGREEMENT

Congratulations on your internship offer! To finalize your placement, you will need to review and agree to the conditions below,
digitally sign and date the form, and send back to your Internship Advisor via email. DCI has secured an internship offer for Vyctori
Anderson at Walter Baker. You have been offered this position on 3/17/2017 and DCI is requesting Participants decision within 24
hours.

Placement Conditions & Acknowledgments

- Participant has full control over accepting or declining this internship offer. DCI is not responsible for satisfaction or
enjoyment of the selected internship.
- Participant receives 1 unit of course credit from Marist College upon completion of their internship and credit requirements.
- Participant agrees to complete a full-time work commitment as communicated by the employer and cannot alter agreed upon
work hours or responsibilities unless employer agrees to recommended changes in writing, in the form of an email from the
employer to the student
- Participant is required to notify the employer and DCI if they must miss work for any reason, including illness. Participant
may not take time off from work without first requesting approval from a DCI Internship Advisor and the above referenced
employer.
- Participant should consult DCI Internship Advisors for advice on how to improve their internship.
- Participant acknowledges that DCI is not obligated to provide replacement.
- Participant acknowledges that if this agreement is contingent on a background check or any additional requirements, and the
Participant has lied or misled either DCI or Walter Baker. There will be no refund of any money paid to DCI. Furthermore,
Participant acknowledges that background checks, if applicable, can take up to but not limited to two weeks to complete.
- If Participant terminates the internship without first consulting with their DCI representative and completing the necessary
steps to improve the internship, as covered in the DCI Internship Coaching Process, with Walter Baker. The Participant
will be dismissed from the program and will not be eligible for financial refund of any kind.
- Participant acknowledges that neither replacement nor refund will be an option if they are fired or removed from their
internship by the employer. Thus, participant may be dismissed from the program and it is the participants responsibility to
notify the impacted parties: i.e. University, Parents.
- Participant understands this internship does not guarantee a job at the completion of the internship.
- Participant understands they shall receive no pay for their internship unless previously agreed upon with the employer.
- Participant understands and agrees to participate with the task or projects described in the internship posting.
- If you are an international student (non-US Citizen) on (insert visa), US State Department restrictions do not allow
you to transfer from one internship to another without updating the visa. Therefore, once the internship program
has commenced, international students are not allowed to change internship.
