1. Balance Convert period is from February 1 to

December 31, 2016. Outstanding balance for
conversion should at least be Php 20,000. NAME

2. To qualify, balance convert application must be

received by BDO within the promo period. It must be
charged on 3, 6, 12,18 and 24 month instalment term. CARD NUMBER

3. Only Principal cardholder can apply his/ her Principal

and Supplementary outstanding balance to this EMAIL ADDRESS
program.
4. Processing is 5 banking days from receipt of the

5. Application forms with incomplete information or no

signature will not be processed

6. Cardholders may inquire the status of their

application by calling BDO Call Center at 631-8000.

7. Outstanding balance applied for conversion will be
multiplied with the corresponding factor rate of the
cardholder's chosen term which will equate to the AMOUNT TO BE
monthly amortization . CONVERTED

8. BDO may, at its sole and absolute option ,

accommodate Cardholder's request to TERMS
accelerate the charging of the full amount of the
remaining unbilled monthly amortization/s on the
Balance Convert instalment transactions, provided the FACTOR RATE
Cardholder's total outstanding balance due for the
pertinent monthly billing cycle is paid in full. Further a
corresponding fee of 5% of the total remaining balance
or Php 300, whichever is higher ,will be charged to the
cardholder's account. After BDO has approved the FIXED MONTHLY
Cardholder's acceleration, the total accelerated amount AMORTIZATION
and the corresponding fee shall form part of the
outstanding balance due for the pertinent monthly
billing cycle and must be paid in full.

By signing on this Balance Convert Application Form , I hereby agree to be bound by the Balance Convert Program Terms and Conditions.
This Program and the other requirements set forth herein as may be amended and supplemented by BDO from time to time and by the Terms
and Conditions governing the issuance and use of the BDO Credit Cards appearing in a separate document. In case of insufficient credit limit,
BDO has the sole discretion to approve only a portion of the amount applied for , without prior notification to the Cardholder. BDO shall have the
absolute and exclusive right to approve or reject applications for Balance Convert. If my Balance Convert application is approved , this shall
serve as my written authorization to BDO to avail the said program . I understand that in case of disapproval , BDO is under no obligation to
notify me of such disapproval and disclose the reasons thereof.
