MYSTERY TRIP 2010

Juniors and Seniors—-

I hope you‟re excited about our mystery trip. It‟s going to be a blast.
We‟re going to have tons of fun AND grow together as a group. As far as
details go, I‟ll be leaving them with your parent‟s as we leave from Del
City. You need to be at the Del City at 3:30p.m. on Thursday. Eat before
we leave! We‟ll return on Saturday at 4p.m. It‟s going to be at great trip!

You need to have 3 items turned in before we leave. If you went on

Mission Trip, you already have a „Trip Release‟, so you don‟t need to fill
another one out.

 Clothes (appropriate length & modest)
 Shoes
 Sunscreen
 Toiletries: soap, shampoo, toothpaste, deodorant,
etc.
 Bath Towel
 Swimsuit (modest) and robe or cover up
 Bedding/sleeping bag (GUYS ONLY)
 Air Mattress that folds up small (GUYS ONLY)
 Your BIBLE
 Pool Towel
 Money for nice lunch on Saturday
 Good Attitude

What Not To Bring

Tobacco products, alcohol products, illegal drugs, fireworks,
matches, Tiny Tim CD‟s, immodest clothing, radios, laptop computers,
game-boys, etc.

Money and Cell Phones:

We won‟t have a ton of time to spend money, so you can bring
as little or as much as you want. You can survive without spending
money, too. Cell phones will be “off” daily, but in the evenings they
may be turned on to call parents only. If misuse occurs, I reserve
the right to take them for the remainder of the trip!

I give permission for my son/daughter to participate in the Del City Youth Group
Junior/Senior Mystery Trip to a decided destination, July 29th-31st. I hereby
waive and release any and all rights, release on claims or damages which I may have
against the Del City Church of Christ and all of their agents, volunteers and
employees, for any and all injuries which my son/daughter may incur while taking part
in your program. I, and my child/ren or ward/s understand the inherent dangers of
daily activities and swimming and I, and my child/ren or ward/s, also understand that
potential injuries include strains, sprains, breaks, cuts, punctures, abrasions, broken
limbs and even accidental death. I HEREBY ASSUME THE RISKS OF INJURY,
DAMAGE OR LOSS WHICH MY CHILD/REN or WARD/S MAY SUSTAIN AS A
RESULT IN PARTICIPATING IN ANY AND ALL ACTIVITIES CONNECTED WITH OR
ASSOCIATED WITH DEL CITY YOUTH GROUP JR/SR MYSTERY TRIP.

This release also encompasses and injuries which may be sustained while traveling to
and from participation in your program. As a parent I understand it is my responsibility
to pick up my son/daughter at the predetermined time. I also understand that if my son/
daughter becomes ill or destructive, the above "EMERGENCY CONTACT" will be
called to take my son/daughter home.
