AFFIDAVIT OF CORRECTION
(Re: Boundaries in TAX DECLARATION No. 2010-04-05-00762)

I, LILIA DIZON ANAYASAN, of legal age, married, Filipino Citizen and

a resident of Liwliw, Camp 4, Tuba, Benguet, after having been duly sworn to
in accordance with law hereby depose and states the following:

1. I am an owner of a certain parcel of land located at Liwliw, Camp

4, Tuba, Benguet and covered and embraced by TAX
DECLARATION No. 2010-04-05-00762 with an area of SEVEN
THOUSAND ONE HUNDRED NINETY NINE (7,199) Square Meters,
more or less;

2. That said parcel of land is particularly bounded as follows:

NORTH E: LIWLIW CREEK SOUTH W: PRECIPICE

EAST SE: MELIZA DIZON WEST NW: RENE DIZON,
AGUSTIN AYADI

3. That the aforementioned property is particularly described in a

Sketch Plan, my lot is LOT 4. Adjacent to my lot is LOT 3 under
the name of RENE DIZON and adjacent to his lot is a CEMETERY.
In the tax declaration particularly West NW is RENE DIZON and
AGUSTIN AYADI, this is erroneous. Perusal of the Survey Plan
adjacent to Rene Dizon is the CEMETERY and not Agustin Ayadi.
Nowhere in the Sketch Plan we could find the name AGUSTIN
AYADI;

4. That I am respectfully requesting the Office of the Municipal

Assessor to correct the boundaries indicated in my Tax
Declaration, particularly WEST NW, from RENE DIZON,
AGUSTIN AYADI to RENE DIZON, CEMETERY, as I am not
aware who that person is;

5. That in support of my request for correction I am submitting a

copy of the Tax Declaration and the Survey Plan;

6. That I am executing this affidavit to attest to the veracity and

truthfulness of the foregoing statement and to support my
request for the correction of my Tax Declaration and for all legal
intents and purposes it may serve.
