AFFIDAVIT OF UNDERTAKING

in accordance with the law, do hereby depose and state that:

legal age, as MPSMU/CPSMU PNCO of __________ station from this date forward
until such designation be terminated for some valid reason/s;

I will adhere to the memorandum issued by the Tarlac Police Provincial Office
that the designated PNCOs for PPSMU will not be subjected for any reassignment.
In case of reasonable necessity of such reassignment, a clearance from the
Chairman of Technical Working Group (TWG) must be acquired first for the approval
of the same to exercise sense of accountability;

I am accountable with respect to the violation of the said memorandum in

case of indiscriminate reassignment of such MPSMU/CPSMU to different section or
stations without acquiring clearance from proper authorities. Further, I am cognizable
at all times with all the undertakings of the PPSMU PNCO, particularly in doing the
assigned tasks and compliances required by the CPSM;

I likewise declare and assume full and complete responsibility for all
consequences which may arise in connection with any misrepresentation, omission
of facts, or violation of this affidavit;

I execute this Affidavit of Undertaking in compliance with the PGS requirement

set forth by the CPSM on the designation of each MPSMU/CPSMU of all stations
province-wide and to attest that all entries thereof are voluntarily conform; and all
violations thereof are clearly understood to the best of my knowledge and belief.
