SENIOR QUESTIONNAIRE

To assist the recommender in representing you as accurately as possible in the letter,


please read and respond to the statements and questions below. The care and thought
you give in addressing these prompts may reiterate what the recommender already
knows about you or may help them discover something previously unobserved. In any
event, take nothing for granted.

1. Explain the qualities that you feel you possess which might easily go
unnoticed by others.
I feel like not many people know of my argumentative side. I feel very capable at
arguing my side in arguments and giving valid evidence to support it. If I was not doing
soccer, I would probably have joined the debate club.
2. Complete the following statement as clearly and directly as you can: I am
especially proud of the following achievement.
I am proud of applying for and being accepted into NHS because it includes a lot
of social volunteer work that I wouldnt typically do in my own time. However, I
overcame that fear and applied for it. In the end, NHS has helped me in the long run
and the volunteering is fun. NHS has helped me become more social and outgoing.
3. Mention any factors that have come up during your high school career
which you feel have put you at a disadvantage.
I take medication for depression/anxiety, and although procrastination is a
common feeling high schoolers have, the side effects of the medication typically make
me drowsy and unwanting to do any of my homework. However, despite this setback, I
still manage to get my work done.
4. Do you feel you present grades are a fair representation of your academic
ability? If not, discuss.
I believe that they do. However, there are some cases where one bad test grade
brings my grade down immensely, so I hope to get those situations fixed. I feel as if I
can make my grades even better if I work harder in my studies.

5. What goals have you set for yourself and what are you presently doing to
achieve them?
I have set the goal for getting a job in the summer. Currently I am researching
jobs that I am interested in and deciding which ones I should apply for. I have also set
the goal of completing my summer homework that I will have. I have begun working on
my AP english homework already by starting to read one of the required books.
6. What activities have you pursued and to what extend? Mention any offices
you have held and awards won. Tell what you gained from this experience.
Ive played high school soccer every year so far. Through this experience, Ive
learned how to be a team player and learn the dynamics of a team. During my
sophomore year on JVB, I was a team captain, so I had to be a leader to my teammates
and try to help them be the best they can. My junior year I made JVA, which has more
competition and is more intense.
7. In what community activities have you participated?
I am in NHS, so there have been multiple community events that I have taken
part in, such as volunteering to help during an elementary schools fun run. Also, during
the school's charity week, I make sure to go to as many of the events outside of the
school that I am able to, like the ice skating.
8. Explain your personal contribution to the high school community.
My personal contribution would be being a good friend to my friends. I also
provide a positive attitude in the face of hardships.
9. What are your hobbies?
Hanging out with friends, drawing, reading, playing guitar, and watching
movies/television.
10. Describe any independent study or special programs/projects in which you
have engaged (science, drama, music serious extra work in a subject.)
I play guitar in my free time and I take some classes to better advance my skills. I
have also traveled on a school trip to California for my film and literature class, which
taught me independence. I advanced my knowledge of the film industry during this trip,
which is something that I am interested in.

11. Describe any work experience you have had paid or unpaid. Describe your
responsibilities, dates of employment, average hours per week, etc.
I typically make money babysitting. My duties involve taking care of the kids and
making sure they are safe during the time that I am there. I usually watch the kids for
over 4 hours, depending on the circumstances.
12. Describe your summer experiences and what you gained from them:
a. After freshman year: I did a soccer summer camp for one week, and I gained the
skill of interacting with strangers.
b. After sophomore year: I went on vacation with my family. I had to watch my siblings
a lot, so I learned how to be independent without my parents.
c. After junior year: I plan on hanging out with my friends a lot more than the previous
summers, which will help me gain more memories and be excited for the future.
