Has the Master written his own Standing Orders and are Bridge Orders being completed and
have the deck officers countersigned them as being read and understood Yes , Signed by all
watchkeepers.

2. Are deck log books and engine movement (bell) books correctly maintained and is an adequate
record being kept of all the navigational activities, both at sea and under pilotage? Yes , all
records such as company check lists, arrival departure tests, pilot boarding time , Tug Fast/cast off , first
line , all cast off, pilots name , c/o con , engaging hand/ auto steering position and time etc mentioned in
the bell book.

3. Are the vessels manoeuvring characteristics displayed on the bridge? Yes as per IMO Res.
A.601(15)

4. Are procedures in place for the testing of bridge equipment before arrival and departure? SOM
C/L 3.03 and 3.06 Complied every arrival and departure along with other check lists as per SOM.

5. Are records maintained of fire and safety rounds being completed after each watch?- Yes ,
Mentioned in the Deck Log Book.

6. Are checklists for pre-arrival, pre-departure, watch handover, Pilot-Master exchange and pilot
card effectively completed? Yes , laminated sheets filled up and recorded in Deck log Book.

7. Does the operator provide guidance on minimum under keel clearance and squat? Yes , UKC
Calculation sheet Prepared for the least charted depth along the route for channel , berth , coastal and
ocean passage taking in to account various factors such as Deepest Draft, sea and swell, hog/sag, water
density, ship squat, tide, list/heel etc and attached to passage plan.

8. Has the Bridge been adequately manned at all stages of the voyage and at Anchor and were
lookout arrangement adequate? Timing of master taken over con , engaging hand steering and name
of look out entered in log book.

9. Is navigation equipment appropriate for the size of the vessel and in good order? All
Navigation equipments working satisfactory , Navtex printer repaired and now printing messages well.

10. Are navigation lights in good order? - Navigation lights has a tandency to get fused frequently.
Light Failure Alarm Working Satisfactory.

11. If a bridge navigational watch alarm system (BNWAS) is fitted is it operational at all times
when the vessel is at sea? - Kept on at all times , connected with hand steering.

12. Are the Standard Magnetic compass and Gyro compasses operational, properly maintained
and adjusted? - deviation found in magnetic compass is larger than mentioned in deviation card.

13. Are auto to manual steering changeover procedures clearly identified? Yes , Posted near
Steering Helm.

14. Are auto to manual steering changeover recorded during periods of river transits and when
navigating through restricted Waters? Yes , Position and Time is recorded when changing over to
auto to manual and vice Versa.

15. Are regular gyro and magnetic compass errors being taken and are they being recorded?
Yes , compass error taken using sun , moon, starts and leading lights as well as jetty alignments where
appropriate.

16. Is there a documented procedure for the operation of the VDR and are the Deck Officers
familiar with procedure to retain the VDR data in the event of an incident? Yes Officers are given
training by master and instructions are also posted near VDR panel.

17. Has a system been established to ensure that all Charts, nautical publications and other
publications are on board, current and maintained up to date? All Charts and Publication on board
are maintained up to date. No Sleeping or inactive folio onboard. Sailing directions ,T&P, Navigational
Warnings, Navtex file maintained and up to date. Cancelled messages have been removed. Chartco
updated every week on receipt of new notice. All corrections and expired T&P notices are applied in
Chortco. Inappropriate practice of updating chart catalogue ( not drawing the boundaries of new charts on
the diagram) and annual summary of notice to marines ( not updating the new editions of sailing
directions) have already been rectified.

18. If the vessel is provided solely with paper charts as an approved means of navigation are all
charts required for the intended voyage of the vessel on board and are these fully corrected?
Full Charts and Publication outfit is up to date to current Notice to mariner.
19. Were the charts used for the previous voyage appropriate? Yes , Appropriate large scale charts
used along the voyage planned route .

20. If the vessel is equipped with an Electronic Chart Display and Information System (ECDIS) as
an approved means of navigation, are the Master and deck watchkeeping officers able to

produce appropriate documentation that generic and type-specific ECDIS familiarisation has
been undertaken? ECDIS is for Training Purpose Only , Training given to all watch keeping officers
and record of the same maintained.

21. Has the vessel been safely navigated and in compliance with international regulations? Yes ,
All relevant information mentioned on the charts such as , PPI, At Least two Method of position fixing,
Watch Level , VHF channels, No Go Areas , Obstructions to be avoided, contingency Anchorages , Abort
Points, Parallel Indexing , Cross Traffic, caution notes,Radar Consp Objects, Notice To E/R , calling
Master , Change of watch Level , Engaging hand Steering, Engine to be on MAN RPM, Removing Anchor
lashing , Posting extra look out if required, PBG, VTS reporting Points And Channels, Port Control Arrival
Reporting Etc.

22. Is the echo sounder recorder marked with a reference date and time on each occasion it is
switched on? Echo marked every time switched on , Arrival port , Departure Port and At Anchorage.
Course recorder marked every noon , arrival and departure . bridge engine telegraph order recorder
marked each arrival and departure.

23. Was a comprehensive passage plan available for the previous voyage and did it cover the full
voyage from berth to berth? Yes , Berth to berth passage Plan Available with all relevant informations
marked on voyage charts.

24. Was position fixing including the use of parallel indexing satisfactory throughout the previous
voyage and the frequency of plotted fixes in accordance with the passage plan? Yes , Position
Fixing complies with SOM chapter 3. Frequency increased where duration found larger than required.
Position compared with radar fixes and Both GPS Compared ,same recorded in GPS log.

25. During pilotage, was the position of the vessel adequately monitored? Yes , Visual , Radar ,
GPS , Comparison of positions

26. Has the GPS been adjusted to the correct datum? WGS Datum 84
27. Is there an adequate system for dealing with navigation warnings and are they being charted?
Vessel receives Navigational Warning Via SAT-C as well as Via Navtex in coastal Areas. All
Navigational Warning Effecting the route charts are being plotted once received. Plotting Date , Time,
Chart Number And date of cancellation if any are marked on warnings with OOWs initials. Additional
navarea can be selected on SAT C to receive Nav warning for the adjacent area vessel likely to pass
through or bound for.
