FAMILY INTRODUCTION
Dominick and AnaLee Addams are an American family that lives in Chicago. They have two girls,

Tanisha age 5 and Harper age 3, who are both happy and healthy. The family was very excited when they

found out they were pregnant. Their little boy came much too early at 24 weeks and 2 days. He was born

a mircopreemie and weighs only 1 Ib. 9 oz. and is 20 cm long. They named him Henry after Dominicks

Dad. He will have to undergo many surgeries and will be at least fifty percent blind due to high oxygen

levels. Due to his extensive health problems, AnaLee only caught a glimpse of Henry before he was taken

by life flight to a childrens hospital 45 minutes away.

Dominick is an air traffic controller and he gets paid $43.28/hr or $84,815 per year. He has been

looking forward to having a little boy, someone he could play sports with and rough house with. He was

also excited to name his son after his father. Dominicks father passed away from cancer just before

Dominick and AnaLee got married. He did meet AnaLee and he approved of their relationship. Ever

since he passed away Dominick has wanted to name one of his kids after his father.

Dominick loves his two little girls and tries his best to provide the world for them. Dominick

does a very good job providing for his family and they live comfortably in the Woodlawn Terrace

apartments at 4726-4740 S Woodlawn Ave Chicago 60615. Their apartment is a 16 minute drive from

their church. The Addams are Born Again Christians and they attend church at the Fourth Presbyterian

Church at 126 E Chestnut St. Chicago, IL.. They go to church at 8:00 am every Sunday so that they can be

sure to hear Matt Helms preaching.

AnaLee is a very active member of the Fourth Presbyterian Church. She attends the mothers group each

Thursday morning. She is part of the LulaRoe team, selling clothing from her home and doing pop-ups

in others homes. She occasionally feels judged by some of her friends who work outside of the home,

but she feels that it is important to be there for her children.

Tanisha loves being a big sister, she helps her mom with everything. She loves to do ballet and

has many friends there. She also goes to mothers group with her mom on Thursday and plays with her

friends. Tanisha is very excited to start Kindergarten, she loves to read and play with baby dolls. She is

very excited to have another baby in the family and to help her mom take care of him.

Harper looks up to her big sister and is a part of the younger ballet group. She was just starting

to understand the idea of her mom having another baby. She loves learning new words and is very

excited to start preschool at her church.

FAMILY STRENGTHS
The Addams family has a strong faith in God. Both Dominick and AnaLee were raised as

Christians and they give credit to the Lord for their financial stability and their family. They have very

strong family relationships. Every Saturday they take time out of their busy schedules and spend 4 or 5

hours focused on family and nothing else. During this time they go on day trips that interest the girls,

play board games, cook together, and just talk. Even before Henry was born they felt that he was a

member of their family. When the doctors asked Dominick and AnaLee if they wanted them to try and

save Henrys life there was not a moments hesitation from either of them. They want their son no

matter what difficulties he may have because they already love him. Family is very important to the

Addams. All of their extended family lives in Chicago and they are very close. Dominicks mom comes

over to babysit when Dominick and AnaLee go on their weekly dates, and AnaLees parents like to take

the girls on outings at least once a month. Every couple of weeks AnaLees family gets together on

Sunday night and has a potluck dinner. Dominick and AnaLee know that if they need anything they can

count on their extended family.

Concern 1: Dominick and AnaLee are concerned about how their sons spastic Cerebral Palsy will affect
him throughout life and have several questions, including:
1. What toys are appropriate for Henry?
2. Are there specifics for how his sisters can play with him? What are the safety concerns with
them holding him or running around in the house when he is on the floor?
3. How can we support his development? Do we need try to get him to crawl as he gets older or
will that harm him?
4. Will he be able to walk on his own or will he need a wheelchair?
5. What adaptations will he need in school?
6. How will this affect his ability to work and have a family?
Current Resource/Information: AnaLee and Dominick have an idea of how a child typically develops
because of their experience in raising their daughters.
Outcome: AnaLee and Dominick will learn more about Henrys development and what they can expect
from him at specific stages throughout his life.
Suggested Action: AnaLee and Dominick will search for answers to their questions using the United
Cerebral Palsy Foundation Website and by using the attached article from that website entitled Each of
Us Remembers.

Concern 2: Dominick and AnaLee are concerned about the possible vision loss that they were told may
come from Henrys constant exposure to oxygen. They are unsure of how to support his development.

Current Resource/Information: They have an optometrist that they have annual check-ups with for each
family member.
Outcome: They will learn more about vision loss and how this will affect Henry throughout his life.
Suggested Action: The family will find more information about how vision loss will affect Henry on the

Rationale: Searching the AFB website, the Addams can find resources for Henry as well as answers to
their questions and a community of support. The information available on this website will help the
Addams to learn about how his vision loss will affect his life and what they can do to promote his
development with accommodations for his vision.

Concern 1: Dominick and AnaLee would like to help Tanisha and Harper adjust to the fact that their
brother has spastic cerebral palsy and understand when Dominick and AnaLee are always running
around to doctors appointments.
Current Resource/Information: Tanisha and Harper understand that their brother is not very strong.
Outcome: By meeting with other kids whose siblings have disabilities, Tanisha will be able to express her
feelings and come to terms with them. By having a meeting that is specifically for her Tanisha will feel
that she is just as important as her brother.
Concern 2: AnaLee is feeling overwhelmed and she doesnt feel equal to the task of raising a child with
spastic cerebral palsy.
Current Resource/Information: AnaLee attends a Moms group with her church, but none of the other
mothers have a child with spastic cerebral palsy.
Outcome: By getting to know other mothers who have children with spastic cerebral palsy, AnaLee will
feel more equal to the task of raising Henry.

Concern1: Dominick and AnaLee would like to know more about physiotherapy, occupational therapy
and speech therapy in their area to help Henry.

Current Resource/ Information: They currently have a basic knowledge of what each Therapist will be
focusing on with Henry.
Outcome: They would like to find the therapists in the area, and understand what that is going to cost
them.
Suggested Action: We are going to help them find therapists in there area

Concern 2: Dominick and AnaLee are concerned with him going to school and would like to know of
preschools, and special education educators and how they will help.
Current Resource/Information: Dominick and AnaLee know very little about the special education
program in their area, they know that their local school does offer a program and they have heard of
IEPs.
Outcome: They will have a basic outlined plan of future schools and programs that are available to their
son.
Suggested Action:

Concern 1: The family would like information on a toy or item that will help improve his ability to crawl
and walk.
Current Resource/ Information: They know about wheelchairs, but they want something that will help
Henry to learn how to crawl/ walk if he is able.
Outcome: The family will obtain a toy or item that will support Henry in learning how to crawl and walk.
Suggested Action: The family will purchase a Scooot Mobility Chair to support Henry as he learns to
crawl and walk.

-The scoot chair is specifically designed to support independence of children with disabilities
that affect mobility. It is typically appropriate for children age 2-6 years old; however, this
depends on the childs physical development, specifically their hip width and leg length
-There are two different models of the chair, the three-in-one and the two-in-one. It is
recommended that the Addams family purchase the three-in-one, which includes the ride
attachment that will help Henry to learn how to use a wheelchair.
-The typical cost for the three-in-one as of June 2016 is $595.00 with free shipping, a 42 day free
return if unsatisfied, and a 12 month warranty.

children with disabilities to have increased independence and mobility so they can more fully
take part in activities of everyday life.

Rationale:
The Scooot Mobility Chair provides independence to children with limited mobility. It will provide Henry
the opportunity to move independently and to practice using motor and cognitive skills involved in
crawling and walking. The Scooot provides support for Henry and the desired outcome through these
areas:
1. Crawling: Engages tummy and shoulder muscles to support children in a crawling position.
2. Scooting: Engages the trunk and pelvis muscles to support children in a sitting position and in
using their legs to move while using their hands to play and interact with their environment.
3. Riding: Engages the upper body and uses movement similar to that which is used to operate a
manual wheelchair and allows the child to move independently and build spatial awareness.

Concern 2: Henry is on oxygen when he goes home from the hospital and the Addams family needs a
stroller/wheelchair that work with an oxygen tank to use during hospital and therapy visits.
Current Resource/ Information: The Addams still have the stroller that they used with Harper, but they
dont know if they will work with Henry.
Outcome: The Addams will get a stroller or wheelchair that work with an oxygen tank.

Concern 1: Dominick and AnaLee would like to know more about physiotherapy in their area to help
Henry.
Current Resource/ Information: They have a basic knowledge of physical therapy and how it will help
their son become more independent.
Outcome: They would like to find the therapists in the area, and understand what that is going to cost
them.
Suggested Action: Dominick and AnaLee will make an appointment with a physical therapist, who will do
a gross motor evaluation and suggest a physical therapist and activities for them to do with Henry at
home.

This office has Pediatric Physical Therapy, they have a unique perspective on maximizing childrens
participation in physical activities. They focus on gross motor movements and suggest to parents
activities to improve performance at home.
Policy Information: Physical Therapy is covered by these insurances, Aetna, Assurant, Blue Corss
Blue Shield, Blue Choice Select POS, Healthlink, Humana Choice Care, Medicare. After the deductible is
met depending on the Coinsurance Percentage it can cost anywhere from $15 up to $55 per visit. The
office is open Monday through Friday 6:30am-7:30pm; Saturday appointments also available.
Appointments can be scheduled up to three times a week. If driving go in to get a parking permit to
avoid parking fees.

Rationale: This resource was chosen to help Henry reach the outcome of his goal to have better gross
motor movements and to provide the parents with more information on Spastic Cerebral Palsy.

Concern 2: Dominick and AnaLee are concerned with finding a speech therapist in their area and would
like to know what their options are.
Current Resource/Information: Dominick and AnaLee know that there son will have speech delays and
they hope by starting him in speech therapy he will be able to express his needs as he gets older.
Outcome: They will find a speech therapist in the area that specializes with children who have Cerebral
Palsy.
Suggested Action: Dominick and Analee will make an appointment with the speech therapist who will do
an evaluation to determine where Henry is and what action needs to be done so he one day can express
his needs.

Program information: PlayWorks offers an initial assessment for the child. The phocus on express
and receptive language. There is an individualized treatment program created for every child. These
appointments can be taken at home, or at the clinic.
The office is open Monday through Friday 9:00 Am to 7:30 PM. This office accepts many forms of
insurance that will help cover the cost of therapy. Appointments are done in the home or at the clinic.

Young Children (Elementary)


The concerns that the Addams have for the younger years with Henry are providing him opportunities to
learn and develop according to his abilities at different stages and knowing what adaptations will be
necessary while he is in school. They will need to work with a specialist to know how to help him to

progress, especially physically, in ways that will not be harmful to him. The family will also be part of the
IEP team in the schools and will need to communicate with teachers and others about Henrys
development throughout his education. They will need to be an advocate for Henry to ensure that he is
getting the support that he needs while in the care of the education system. Also, they may consider
home schooling if the school system is uncooperative or does not seem like an appropriate environment
for Henry.

Families With Adolescents


The Addams family is concerned about Henrys interactions with peers. As he enters his middle and high
school years, social interaction with peers becomes more important, and the family is worried that, with
the differences in Henrys development as compared to peers, he will not have the physical abilities to
participate in the same activities as his peers. Also, if they choose to home school, they are concerned
about providing opportunities for peer interaction. The Addams will need to look into support groups
and find other families in the area who have children with similar disabilities in case the interactions at
school and in the neighborhood are not beneficial or safe for Henry.

Families Nearing the End of Life


The Addams do not know how independent Henry will be as they approach the end of their lives and as
he reaches adulthood. They are concerned about finding someone to care for him if he is not able to live
independently and ensuring his quality of life is high even if he is able to live independently. They are
unsure if he will be able to be married and have a family of his own and do not know how many lasting
friendships will come throughout his life. The family will support Henry in building lasting relationships
with others through support groups and educational endeavors. They need to consider who would be
willing and able to support Henry until the end of his life if he ends up being unable to care for himself.
