BEFORE ME, this day personally appeared NEIL GILLESPIE, who upon oath deposes
1. I am over the age of eighteen and am competent to testify as to the facts and matters set
forth herein. I make this affidavit upon personal knowledge unless otherwise expressly stated.
3. Psychologist Selina Kassels, Ph.D., License No. PY5229, did an initial psychological
evaluation of me on 09/04/1998 and 9/11/1998 at the Florida Center For Cognitive Therapy, Inc.
4. A true and correct copy of Dr. Kassels' four (4) page initial psychological evaluation of
me is attached to this affidavit. (The original evaluation appears on blue paper).
5. Selina Kassels, Ph.D. is now deceased, according to the attached Florida Department of
Health license verification page for Dr. Kassels, License No. PY5229.
6. I wrote to Dr. Kassels on September 19, 1998 about medical records and HMO coverage.
7. I faxed Michael A. Greif, Senior Attorney for the Division of Vocational Rehabilitation,
(DVR) on October 8, 1998, RE: Gillespie v. Florida DVR, DOAH Case No. 98-3444, informing
him Dr. Kassels and the Florida Center For Cognitive Therapy, Inc. were licensed DVR vendors.
8. I faxed Dr. Kassels on October 8, 1998 in part, "After speaking with Mr. Greif...I believe
we have an understanding that DVR will fund an assessment and possible treatment."
10. Copies of five (5) co-payment receipts from the Florida Center For Cognitive Therapy,
Inc. are attached for my sessions with Dr. Kassels, each for $20, as follows:

AFFIDAVIT OF NEIL GILLESPIE
11. A Statement for Professional Services dated February 18, 1999 from the Florida Center
For Cognitive Therapy, Inc. for my sessions with Dr. Kassels is attached.
12. I wrote Dr. Kassels on October 30, 1998 about a paper titled "Psychotherapy for Persons
with Craniofacial Deformities: Can We Treat Without Theory?" by Bennett & Stanton.
13. I wrote Michael Greenberg Ph.D. on November 13, 1998 about continuing treatment.
14. I was notified on November 25, 1998 that Dr. Kassels was on a medical leave of absence
by letter ofM. Locascio, office manager at the Florida Center For Cognitive Therapy, Inc.
15. On December 28, 1998 Dr. Kassels wrote me in part, "I have returned from medical leave
and am resuming my practice at Florida Center For Cognitive Therapy ..."
16. I received a letter March 31, 2004 from Selina Kassels, Ph.D. in response to my letter on
behalf of "Mary", a lady and mother whom I referred to Dr. Kassels for therapy.
17. I am no longer suicidal as Dr. Kassels wrote in her initial psychological evaluation of me.
Ultimately I resolved that issue when I found my calling as an advocate. I decided it was better
to die in service of a cause, for others, to have a good death, to bear witness, to go out fighting.
18. This affidavit replaces my affidavit made May 4, 2017 after I found another file with
more information for Selina Kassels, Ph.D. and the Florida Center For Cognitive Therapy, Inc.

Initial Evaluation
Patient reports that he is currently attending SPJC, pursuing studies for a legal
assistant and that he is also working as an independent consultant doing legal
research. He has a significant medical history and during the past few years
devoted much of his time and energy to obtaining a complex surgical
restoration and repair for a cleft palate. He is currently frustrated that his
medical insurance is not authorizing his request for replacement of part of his
prosthesis. He is also having some uncertainty about his vocational trainitlg
and was asking the administration at SJCP to address his concerns about ail
ethics course in which he felt the instructor was not teaching the course as
described in the curriculum information.

anxiety symptoms and panic attacks and ties these symptoms to his life-long
experience of living with a disability, a cleft palate--an experience he
describes as "living Hell...constant verbal and physical abuse."
Patient was raised in Bucks County, Penn. His father was a computer
programmer who eventually was technologically unemployed in that field due
to lack of specialized education. The patient has a younger brother who was
in the military and a younger siser who was married and divorced and now
lives with his parents. He describes himself as having "strained
relationships" with his family while he was growing up. He is currently
alienated from his family of origin citing his pursuit of both higher education
and medical care as the reasons why he no longer can communicate with
them. He believes they are unable to support his goals because no one else in
the family has education beyond high school. Also he believes that his
pursuit of medical care now is perceived as "reflecting poorly" on them for
not advocating previously for corrective care during his childhood.
After graduating from high school, patient worked for US Steel and then in
restaurant maagement and car sales. In 1980 he opened his own dealership
which he owned until 1991. Eventually he became very successful financially
and "sold the business for $2 Million dollars"; but then he reinvested in
another car dealership in Philadelphia where he lost the money. During the
time he was in business he employed/or assisted some of his family, stating
they were there "during the good times." He relocated in May of 1993 to the
Tampa Bay area with other family members. After moving to Florida, he was
evaluated psychologically and vocationally through the Office of Vocational
Rehabilitation and reports he is now receiving disability. Patient moved to
Oregon a few years ago to avail himself of the lengthy restorative surgery
with a specialist in cleft palate repair whom he had consulted with in St.
Petersburg. While in Oregon, he completed a B.A. degree in philosophy and
.after spending some additional time on the West Coast he recognized that the
rain contributed to his depression. He packed his belongings in a van, spent

time "on the road" in California searching for a cite to relocate and after his
van broke down, he returned to St. Petersburg where he now lives by himself.
Patient reports that he had seen a psychiatrist in Philadephia for two years,
Dr. Wainwright, and found that treatment helpful in mobilizing his energy to
focus on reconstructive surgery. He did not wish to authorize release of
previous psychological evaluation at this time. He also speaks of that therapy
addressing "relationship issues". He reports that he contemplated marriage
and that his fiance's family opposed the match. He has not had a serious
relationship in ten years and no longer thinks about any desire to marry. He
has a very strong conviction that he is not meant to "procreate".

Concerning the provision of my medical records, I have decided not to release them at this
time. After considering the matter, I do not believe it would facilitate my current
objectives. In addition, until the HMO approves more sessions, it may be futile.
Regarding the HMO and approval for treatment, I am concerned about scheduling more
appointments until this matter is resolved. What happens ifwe meet, and then the HMO
declines payment? Am I responsible for the full fee? Please advise.
I spoke with Amy on September 17 (the day preceding my last session) inquiring ifmore
sessions had been approved by the HMO. She assumed they did, saying you would have
contacted me otherwise. Because of past problems I encountered with the HMO and its
failure to provide treatment approval, I want to resolve this issue before meeting again.

photo, I gained significant weight (about 100 pounds). Also attached is a letter
from Dr. Li inviting me to work at his hospital in PRC. I want the photos back,
and ask that you return them on October 20 when we meet next.
As you can see, there is quite a bit ofinfonnation here. In addition, I authorized Mr.
Grett: the attorney for DVR, to release the entire DVR file to you. I believe it's over 500
pages. And for a more detailed look at the process, I've included a copy of my Second
Amended Petition for Administrative Hearing, which while not in active litigation (the
Third Amended Petition is, copy enclosed, without exhibits), it should give you a historical
account of events from my perspective.
Ifyou need more time to review this material before our next appointment, perhaps we
should reschedule until after you have had time to reviewed the documents.

Dear Neil,
I am writing in response.to your recent letter. First, I want to thank you for thinking about me and
sending nte such a kind letter. I ~ very grateful to hear that you thought I was a good therapist.
It is always helpful and pleasant for me to get such kind feedback.
Since Mary does not drive and needs help it is important that you locate social service agencies that
might be able to send 'someone out to-,her home to help her. Once she is cOlUlected up with the
appropriate social service agency in the area where she lives, it is likely that the social service
agency can help her find a good therapist and possibly arrange for transportation.
I hope things are also coming along for you. Take good care of yourself..
