(c) Copy of GATE score card/CSIR/UGC NET for JRF/DBT/ICMR JRF/
certificate, if qualified.

SPECIFIC INSTRUCTIONS FOR FILLING IN THE APPLICATION FORM FOR

THE Ph D PROGRAM :

1: Name Fill in your name, as recorded in the qualifying Degree certificate issued by your
University/Institute, in CAPITAL LETTERS. Your name will appear in the Institute
records in the same manner as filled in by you. Any change in the name/surname
at any later stage has to be duly supported by a proper affidavit.

2: Gender Put a tick mark in the appropriate box.

3: Nationality Put a tick mark in the appropriate box.

4: (a) Category & (b) Sub Category Put a tick mark in the appropriate box(es).

5: Date of Birth Enter your date of birth as given in your High School (Class X) Certificate
in the space provided. Example: If your date of birth is 9th December, 1982,
fill in as 09/12/1982.

6: State of Domicile Write the State of your permanent residence.

7: Phone Number Give your contact phone number in the box provided along with the
STD code.

8: Employment Status Put a tick mark in the appropriate box.

9: Address Write your address for communication including pin code.

10: Your email address Write your email address legibly.

11: Degree Code Write the abbreviation of your qualifying or final year of your degree.

University Code Write the University name from where you have obtained or will
be obtaining your qualifying degree

Branch Write the branch of your qualifying degree.

Status of qualifying degree Put a tick mark in the appropriate box depending on whether
you have passed, appeared or appearing for the qualifying
examination.
 Year of qualifying degree Indicate the year of passing / appearing/ appeared for the
qualifying degree.

Marks obtained in qualifying degree Indicate the percentage/GPA scored in your

Qualifying degree (upto 3rd year in case of
BE/B Tech, and 1st year in case of M Sc).

Class obtained in qualifying degree Indicate the class obtained in the qualifying degree.
If your University uses GPA and does not award a
class, leave this entry blank.

12: GATE Indicate your GATE 2008/GATE 2009 particulars.

13: Put a tick mark in the appropriate box if qualified or appeared and write the roll number of any
one of the tests which has validity for admission. Candidates with valid certificate in one of the
tests will be short listed for the interview. (Qualifying CSIR/UGC NET for Lecturership will
NOT be considered for short listing for interview). If you have qualified in more than one tests,
the details may be given in the Academic Profile appropriately.

14: IISc Entrance Test 2009: Fill in this column only if you have already qualified and called
for the interview at IISc during June 2009.

15: Preferences of Departments Candidates may choose upto 3 departments/centres/units

from the announced list.

Fill in all the entries in the Academic Profile appropriately.
