AFFIDAVIT
I, MYRELENE D. GANADIN, Filipino, married and a resident of Sampao, Isulan,
Sultan Kudarat after having been duly sworn to in accordance with law, do hereby depose and
say:

1. I have then to applied for a Philippine Passport bearing No.UU0657377 before the
Office of Department of Foreign Affairs (DFA)- Davao City, Sometime on March 5,
2007, ;

2. Sometime on January 8, 2012 my employer applied for an extension with renewal of

my Philippine Passport at the Philippine Embassy of ABU DHABI. However the
above-mentioned Passport was not released/given to me until such time I went home
to the Philippines on February 2, 2012;

3. Sometime on August 29, 2012. I again applied for a new Philippine Passport bearing
before the office of the Department of Foreign Affairs (DFA)-General Santos City.
As I am about to claim my Passport, I was informed by he said office that I have an
unclaimed passport;

4. By the reason of the foregoing, the same resulted to a double issuance

/registration of my Philippine Passport with the Department of Foreign Affairs
(DFA)- General Santos City which I never deliberately/intentionally done or made;

That I execute this affidavit to cancel one of my Philippine Passport bearing No.
UU0657377 and to attest the truth of all the foregoing facts for all legal intents and purposes.

IN WITNESS WHEREOF, I have hereunto set my hand this 27th day of November,
