BOARD RESOLUTION No. 8

2013 VEHICLE STICKER SYSTEM (VSS) and STRICT CONTROL OF
VISITOR ENTRY IN THE SUBDIVISION

WHEREAS, Article I, of the by-laws states that the associations primary concern is to
facilitate the delivery of adequate social services and economic advantages for the association to
improve the quality of life and well-being of its members.

WHEREAS, Article X, Section 1 of the by-laws, provides the Board of Directors the
powers to promulgate rules and regulations consistent with law, the articles of incorporation and
the by-laws. Unless provided otherwise in these by-laws, all policies, guidelines, rules and
regulations promulgated by the Board of Directors concerning the imposition and collection of
fees, dues, charges, and other assessments, or the use of the facilities and services rendered by
the Association, or the imposition of any sanction against members of the Association, shall take
effect only at least ten (10) days after the official posting thereof on the bulletin board of the
Association, or the receipt of the written notice by the member concerned; and,

WHEREAS, the board has learned among neighbor subdivisions, numerous incidents of
trespassing, robbery and the likes, and the board deemed it necessary to augment the
subdivision security by strictly controlling and identifying the entry of visitors inside the
subdivision with the following guidelines;

1. 2013 VEHICLE STICKER Benefits, Fees and Application Form:

Benefit:
To Vehicle Sticker owners: vehicles may enter the premises without the need to
leave ID or Drivers License to the guard.
To residents: controlled and identified entry of all visitors in the subdivision
To guard: Visual control for easy identification of vehicles for residents and non-
residents.

Fees Applied:
For the 1ST vehicle:
FOR HOMEOWNERS PHP25/wheel of vehicle
FOR RENTERS/LESSEES PHP25/wheel of vehicle
FOR OUTSIDERS* PHP50/wheel of vehicle
*OUTSIDERS ARE SUPPLIERS, VENDORS, SCHOOL SERVICE AND OTHERS

From the 2nd vehicle onwards: ADDITIONAL PHP 10 / wheel of vehicle

Incentive: by next year 2014 ALL MEMBERS WHO PAID THEIR MONTHLY DUES
CONSISTENTLY FOR 12 CONSECUTIVE MONTHS ( May 1, 2013 to April 30)
2014 shall receive ONE (1) FREE STICKER.

2. COVERAGE - All vehicles including motorcycles, and PUVs owned and operated by the
Homeowners and is parked in their respective premises. All outsiders recommended by
the homeowner such as renters, suppliers, school service, water station and others.
Heavy vehicles such as 10 wheelers, buses and the like will be prohibited from applying
for a sticker and not be allowed to enter the gate.

3. REQUIREMENTS - Present duly accomplished Application Form with signed undertaking

together with photocopies of the following requirements:

Photocopy of Official Receipt (OR) and Certificate of Registration (CR).

Photocopy of duly notarized deed of sale, if registration is not in applicants name.

Photocopy of Sales Invoice & Delivery Receipt for newly acquired vehicles
Certification from the company, if company-owned vehicle.
Vehicle rental agreement, if rent-a-car
For Renters:
o Photocopy of updated Contract of Lease, if contract of Lease on file has already
lapsed
o Endorsement from the homeowner (homeowner must sign the application form)

4. Only PHSHOA authorized representative shall install the sticker on the applicants vehicle.

5. Stickers are NON-TRANSFERABLE.

6. Stickers installed by temporary means like the use of scotch tape will automatically be
confiscated by PHSHOA security.

7. The vehicles owner shall be equally and directly liable for all violations of the PHSHOA
traffic rules and regulations committed by his/her authorized driver.

8. The issuance of vehicle sticker does not carry any acceptance of liability on the part of
PHSHOA.

9. All vehicles are subject to the PHSHOA rules and regulations and barangay traffic
ordinances.

10. All stickers not claimed w/in Sixty (60) days from date of scheduled release will be
forefeited.

11. SCHEDULES Last day of sticker application will be on May 25, 2013
The vehicle sticker is valid until May 31, 2014. All vehicles without sticker must leave
drivers license at the guard, effective June 1, 2013 and onwards.

12. VISITOR/S OF HOMEOWNERS PHSHOA shall provide all homeowners with a Visitor
Advise Form to be filled up by the homeowner as an advise to the guard that they will
have visitors coming. All visitors without vehicle sticker are subject to interview, log in and
verification process by the guard.

Requirements : 12.1. Presentation of ID (for visitors without vehicle) and drivers

license (for those with vehicle) and mention the name, block and lot
of the homeowner to be visited.

12.2. Guard shall check if there is a Visitor Advise Form issued

by the homeowner.

12.3. If there is no Visitor Advise Form issued, the guard shall

request the visitor to inform the homeowner to pick them up at the
gate. If there is no means of communication, the guard shall go to
the homeowners house and ask the homeowner to pick up the
visitor or fill up a Visitor Advise Form

13. GATE PASS all entries in PHS will be issued gate pass. Lost gate pass is subject to
verification and a fee will be imposed:

Fees Applied : PHP 100.00 lost gate pass

Pass Codes : V visitors; H homeowners; T tricycle; D- delivery;
S school bus

14. Public Utility Vehicles or PUVs are subject for inspection of the guard and needs to be
issued Gate Pass while LATODA tricycle units are exempted from from both PHS 2013
Sticker and Gate Pass.

WHEREAS, the Board of Directors shall implement the above provisions effective 1 June 2013;

RESOLVED THEREFORE, that the HOA, through its Board of Directors, hereby APPROVE AND
ADOPT the above specified provisions;

RESOLVED FINALLY, that the notice of this resolution be furnished to all members of the
Association for their information and guidance;

ADOPTED and APPROVED this 22 April 2013, at Dasmarinas City, Cavite, Philippines and this
resolution shall supersede any and all previous resolutions and/or memo and/or circulars
issued by the PHSHOA pertaining to Vehicle Sticker and Control of Visitor entry in the
subdivision.
