AFFIDAVIT OF INCOME

I, Mah Tuh Ling, of legal age, Filipino, and presently residing at Sitio Madulas, Brgy. Nadapa,
Tarlac after having been duly sworn to in accordance with law, depose and state:

- I have been engaged in the business of selling merchandise.

- The business is now firmly established ever since I have started it a couple of years ago.

- Having been engaged in this business for a long time, I already have been earning income in the
amount of approximately THIRTY THOUSAND PESOS (PHP 30,000.00) a month.

- I therefore have executed this Affidavit to attest to the truth I mentioned herein and for whatever
legal purposes it may serve.
