1. Summarize Performance Evaluation based on Phase III (Performance Review and Evaluation).
Refer to the MS Excel Template

2. Accomplish this 3-part consolidation of Development Planning aka


Individual Plan for Professional Development (IPPD)
a. Phase IV Consolidation Form 1
b. Phase IV Consolidation Form 2
c. Phase IV Consolidation Form 3

3. Accomplish accomplishment reports on training, mentoring and coaching using GROW model
In compliance with Division Memo 267 as part of Phase II (Performance Monitoring and
Coaching)

4. Submit the Phase IV Consolidation Forms 1 to 3 to Regional Office and Central Office
5. This report serves as Compliance Report to
(a) CSC and (b) DBM for the PBB and Mid-Year Bonus
