Outstanding Mindanao Area Geodetic Engineer of the Year Form

Outstanding Member:
The Outstanding Award recognizes a GEP Member whose outstanding services and
dedication to the organization enhances the mission of GEP. The recipient of this award
is a GEP member who went above and beyond the call of duties and responsibilities
expected of a GEP member or board of directors.

Eligibility:
GEP member in good standing for the last two (2) consecutive administrative year 2012-
2013 & 2013-2014. An eligible GEP member will be considered for the Outstanding
Mindanao Area Geodetic Engineer of the Year upon nomination and if he/she fulfilled
the following:

Must attended or participated in at least five (5) of the local chapter or regional
activity;
Must have no unpaid account or pending obligation to the GEP organization;
Must be of good moral character (supported with the following)

Barangay Certification as a man of good repute

Police or Fiscal Clearance

Must be endorsed or nominated by the chapters Board of Directors attested by

the Chapter Secretary with accompanying write-up of achievements place in a
long folder and;
Professional Profile.

Nomination Procedure:
Nominator must be a member of good standing

Submit a nomination letter that comments on candidates service and

dedication to GEP.
Self-nomination is accepted. The self-nominee should submit a letter of support
from the fellow GEP member.

Candidate or nominator must complete this form and send it with other required items
to Engr. Voltaire Anthony C. Rosario @ ADFIL Corporation Doongan, Butuan City.
Deadline for submission is on October 31, 2014.
