Safe Work Instruction 01 Working @ Heights/EWP/Man Cage/Confined Space

1 Working in a Man-Cage Falls from heights Ensure that all occupants have completed a working at heights course and that they are competent
Ensure that the Rigger who is in control of the man-cage is licensed, competent and VOCed for this site
Obtain the correct permit for the job and ensure a risk assessment has been completed with the JSA
100% Hook-up must be used at all times whilst using the Man-Cage
Dropped objects Tool lanyards are to be used
Chin straps must be used at heights
Barricade all drop zones and use adequate signage along with information tags
Equipment Failure Inspect all lifting gear on the Man-Cage and inspect the Man-Cage is suitable for the task
Crane operator is to ensure that the crane is capable and suitable for the task
Crew is to check and monitor wind and weather conditions
Radios are to be used for communications and for crane signals at ALL times
Ensure radios are charged and operational with correct channels available
Safety line between hook and man cage rigging must be used

2 Working At Heights Falls from Height All personnel MUST be trained, deemed competent and VOCed
Ensure that a WAH permit has been issued and the controls and restrictions are in place
Ensure that the harness is fitted correctly, quarterly inspected and tagged and inspected prior to use
100% hook up must be used whilst working at heights
Lanyards must be hooked on overhead where possible
Do not hook directly onto grid-mesh and handrails
Ensure anchor point is capable of withstanding 1.5ton for single person, 2.2ton for two people
Use beam straps with protection in place on sharp edges
Do not exceed 30 radius for inertia reels
Static lines and Inertia reels to be installed by trained personnel
Dropped Objects Tool lanyards are to be used
Barricading to be erected for drop zones and to isolate area
Ensure information tags are placed on barricading

3 Work from EWP/Scissor Lift Machine faults Conduct pre-starts on machinery and record data in log book
Ensure rescue procedure is developed
Ensure fire extinguisher is charged, serviceable and within 6 monthly inspection period
Uneven ground EWP/scissor lift must be setup on even ground
Ensure services are identified that may pose stability issues
Ensure when travelling in EWP that it is conducted with the basket lowered and on turtle mode, in addition use a spotter
Noise Ensure hearing protection worn where required
Noxious Fumes Ensure machine is used in a well ventilated area and fumes dont hinder other work groups
Unauthorised machine use All personnel MUST be trained, deemed competent and VOCed
Falls from Height Ensure that the harness is fitted correctly, quarterly inspected/ tagged and inspected prior to use

Ensure harnesses are attached to a designated anchor point whilst inside EWP/scissor lift at all times
Dropped objects Tool lanyards are to be used
Chin straps must be used at heights
Barricade all drop zones and use adequate signage along with information tags
Congested areas Spotters are to be used in congested areas or where there is potential for contact with structures and equipment
Establish communications with other work groups

4 Exiting EWP/Scissor Lift/Man Cage Unintentional machine movement Ensure basket is sufficiently secured before exiting basket
Turn machine off before exiting basket to avoid accidental operation of controls
Falls from Height 100% Hook-up must be used at all times
Tool lanyards are to be used
Chin straps must be used at heights
Barricade all drop zones and use adequate signage along with information tags

5 Grid Mesh Removal Falls from Height All personnel MUST be trained, deemed competent and VOCed
Ensure that a Grid Mesh removal permit has been issued and the controls and restrictions are in place
Hard barricades are installed around area before grid mesh is removed
Ensure signage has been erected
Ensure that the harness is fitted correctly, quarterly inspected and tagged and inspected prior to use
100% hook up must be used whilst working at heights
Lanyards must be hooked on overhead where possible

Manual Handling Injuries Use correct lifting techniques when shifting, lifting, moving equipment
Plan all manual handling lifts (path to be travelled, within personal capabilities)
Dual person lift where necessary
Use mechanical aides where possible

6 Confined Space/s Hazardous Energy/Atmosphere Isolations must be complete before work commences
Permits must be in place before work begins
Gas testing must be conducted by a suitable qualified person
Faulty Equipment Gas tester must be within calibration date
Unidentified Confined Space Ensure signage is erected and maintain throughout duration of task
Barricade area if required and place information tags on barricading
Dark or unlit work area Establish sufficient lighting within space
Ensure lighting is extra low voltage (32v)
Communication Sentry to be in place for duration of occupancy of space and be trained/competent
Radio communication to be established where possible between occupants and sentry
Radio for Sentry for emergency services and supervisor contact

Hazardous Atmosphere Ventilation of space to be provided where required (Hot works)
Purging of atmosphere required where space has been filled prior or where required
Safety line to be used where deemed necessary
Supplied air respirator to be used where deemed necessary
Noise Class 5 hearing protection to be used
Uneven surfaces, material in area Use correct paths to access work fronts
Remove excess materials in area
Dispose of unwanted materials
Notify supervisor if remediation work is required
Maintain housekeeping throughout task
Remove combustible materials
