 Fitness For Duty Determination
Fit Criteria - General

1. Hb level above 10 gr/dl

2. No evidence of active infectious disease such as Tuberculosis, Viral
exanthema, etc.
3. No evidence of renal failure, recurrent UTI (except after successful
treatment), BPH with urethra; stricture interfering complete bladder
evacuation for remotes/offshore workers
4. No current clinical diagnosis of myocardial infarction, angina pectoris,
coronary insufficiency for remotes/offshore workers and Heavy type
work
5. Post CABG/PTCA has free of symptoms for at least 3 months and proven
by treadmill no ischemic response to Bruce protocol stage 3 and with
optimal medication and routine control for remotes/offshore workers
6. No Epilepsy event within the last 2 years
7. No evidence of malignancy disease
8. No evidence of mental disorder not limited to depression, alcohol
dependence, psychoses, phobias, and anxiety disorder

Non-threatening coronary anatomy finding (>50% LAD, LM, and 3VD)
No ischemic response in treadmill
Ejection fraction >40%
Optimal medication and routine control
Congenital Heart Disease
No symptoms and no hemodynamic function failure as stated by
cardiologist proven by no ischemic response to Bruce protocol stage 3

(i) A hand or finger which interferes with prehension or power grasping
(ii) An arm, foot, or leg which interferes with the ability to perform normal
tasks associated with operating a commercial motor vehicle
(iii) Any other significant limb defect or limitation which interferes with the
ability to perform normal tasks associated with operating a commercial
motor vehicle
DIABETES
Has no established medical history or clinical diagnosis of diabetes mellitus
currently requiring insulin for control

CARDIOVASCULAR
Has no current clinical diagnosis of myocardial infarction, angina pectoris,
coronary insufficiency, thrombosis or any other cardiovascular disease of a
variety known to be accompanied by syncope, dyspnea, collapse or
congestive cardiac failure

RESPIRATORY Has no established medical history or clinical diagnosis of a

respiratory dysfunction likely to interfere with ability to control and
drive a commercial motor vehicle safely
There are many conditions that interfere with oxygen exchange and
may result in incapacitation, including emphysema, chronic asthma,
carcinoma, tuberculosis, chronic bronchitis and sleep apnea
EPILEPSY Has no established medical history or clinical diagnosis of epilepsy or
any other condition which is likely to cause loss of consciousness or
any loss of ability to control a motor vehicle

MENTAL Has no mental, nervous, organic or functional disease or psychiatric

DISORDER disorder likely to interfere with ability to drive a motoR vehicle safely

 Driver Fitness For Duty Determination

DRUG USE Does not use a controlled substance such as an amphetamine, a narcotic,
or any other habit-forming drug.

Exception: A driver may use such a substance or drug, if the substance or

drug is prescribed by a licensed medical practitioner who is familiar with
the driver's medical history and assigned duties; and has advised the driver
that the prescribed substance or drug will not adversely affect the driver's
ability to safely operate a commercial motor vehicle

ALCOHOLISM Has no current clinical diagnosis of alcoholism
