COMMISSION ON AUDIT Commonwealth Avenue, Quezon City MEMORANDUM FOR All Assistant Commissioners, and All Others Concerned This Commission rectors, Officers-in-Charge fy FROM Assistant Commissioner CARMELA Sere Chair, Central Office Steering Committee SUBJECT : Submission of a Certified Matrix as attachment for the SPCR and OPCR with Actual Accomplishments and Ratings DATE March 2, 2017 In connection with the Technical Working Group, SPMS Meeting held last February 21, 2017, it was agreed that in lieu. of the supporting documents as attachments to the Sector / Office Performance Commitment and Review Form (SPCRIOPCR) with actual accomplishments and ratings for the Second Semester, 2016, may we request a certified matrix (sample format attached) showing the quantity accomplished and date of received and complied as stated in the Success Indicator. This is to facilitate the evaluation of the said forms in view of the deadline on the submission of Form 1 (Result of Ranking of Delivery Units) as requirement of the AO 25 for the CY 2016 Performance-Based Bonus (PBB) In the meantime, we will consider the supporting documents which were already submitted by the Sectors / Offices. However, for those which have incomplete / without supporting documents, please submit a matrix certified by the Office of the Assistant Commissioner (OAC) on or before March 8, 2017 to the Risk Management and Budget Office (RMBO). Otherwise, outputs without certified matrix will not be considered / rated but included in the total number of outputs of the Sector / Office which may affect the Sector / Office rating. Also, validation/random sampling of data may be done at the concemed offices only for confirmation purposes, when necessary.
